Pediatric epilepsy, a chronic, recurrent brain disorder, is the most common neurological disorder in children. Its prevalence is increasing. Early management is very important since 30∼40% of cases persist into adulthood. To provide basic data for future clinical research on pediatric epilepsy using Korean medicine treatment and cooperation between Western medicine doctors and Korean medicine doctors, we reviewed recent clinical research in traditional Chinese medicine (TCM) using herbal medicine for pediatric epilepsy. A total of 23 articles (1 clinical practice guideline, 3 systematic reviews, 15 randomized controlled trials (RCTs), and 4 non-RCTs) were reviewed in this study. The authors summarized characteristics of included studies regarding study subjects, diagnostic tools, pattern identification tools, treatment period, evaluation tools, detail of herbal medicines, treatment effects, and adverse events. Combination therapy using both herbal medicine (HM) and anti-epileptic drugs (AEDs) was performed more frequently than herbal medicine alone. Liver-pacifying medicinal, water-draining medicine, and orifice-opening medicine were frequently used. The main single HMs were Cheonma, Boglyeong, Jogudeung, and Seogchangpo. Combined therapy using HM and AEDs had significant benefits in improving total effective rate. It also appeared to be safer than AEDs. However, since the quality of clinical trials was poor and only studies in the last 10 years were included, the clinical evidence was uncertain. Finally, the authors provided limitations of this study and several suggestions for future research based on our analysis results.

Objectives: To understand experiences of daily life and Korean medicine treatment process and perspectives of caregivers caring for children with tic disorder.
Methods: A semi-structured interview was conducted with caregivers of patients under 13 years who were experiencing Korean medicine treatment for tic disorder. A total of five participants were interviewed. The analysis was performed according to a four-step phenomenological method.
Results: The following four components were derived from the experience of caring for children with tic disorder: general perception of tics, negative emotions caused by tics, social deprivation, and living with a child with tics. The following four components were derived from Korean medicine treatment experience for tic disorders: criteria for selecting treatment, awareness of Korean medicine, treatment experience in Korean medicine, and what needs to be improved.
Conclusions: According to results of this study, caregivers caring for children with tic disorder experienced negative emotions such as frustration, guilt, and anxiety. They also experienced social deprivation such as stigma and prejudice. Korean medicine treatment can be an effective alternative for improving symptoms of tic disorders. The application of insurance benefits to Korean medicine is needed for patients and caregivers to autonomously choose treatment.

Objectives: For diagnosis and evaluation, evaluation tools are needed. Various tools can be used to diagnose and evaluate mental disorders. Among them, psychological tests are widely used. For Korean Standard Classification of Diseases (KCD) diagnosis, psychological tests are also required. Currently developed critical pathway (CP) presented tools for diagnosis and evaluation of mental disorders. The CP suggests the use of tools based on the Clinical Practice Guideline (CPG). Therefore, CPG-based tools should be able to be used in the clinical scene of Korean Medicine for diagnosing and evaluating mental disorders according to CP.
Methods: Tools suggested by CPs are summarized. The degree of utilization of tools in CPGs is also summarized. A review was conducted by Korean Medicine neuropsychiatrist experts on tools and user's usage plan.
Results: As a result, developed CPs suggested using 19 tools for anxiety disorder, 13 tools for insomnia), 12 tools for Hwabyeong, and 9 tools for dementia. In CPG, 48, 34, 44, and 44 tools were used for anxiety disorder, insomnia, Hwabyeong, and dementia, respectively. Among tools presented in CP, HAM-A, HAM-D, CGI, SAS, and TESS for anxiety disorder, CPG, ISI, and PSQI for insomnia disorder, CPG, STAI, and STAXI for Hwabyeong were frequently used in CPG. For dementia CPG, MoCA, MMSE, HDS, ADL, and ADAS-cog were frequently used. Among them, MoCA, ADL, and ADAS-cog were suggested tools in CP.
Conclusions: As a result of analyzing tools suggested and used in the developed CPs and CPGs, it was verified that various tools were used in each study. Most of them were symptom and behavioral evaluation scales. Therefore, symptoms and behavior evaluation scales used more frequently should be able to be used in the clinical scene of Korean Medicine.

Objectives: Depression is a highly prevalent disease, and the market for antidepressant drugs continues to grow at a steady rate. Although current antidepressants are reported to be effective, because of their low remission rate and side effects, new antidepressants are needed. The use of Banhahubaktang (BHT) to treat the symptoms of depression is supported by experimental evidence. The proposed study will evaluate the efficacy and safety of BHT in treating depression.
Methods: A randomized, waitlist-controlled, parallel clinical trial will be conducted to assess the efficacy of BHT in depression. A total of 84 participants with depression will be randomized into the intervention group or waitlist-control group at a 1:1 ratio. Patients in the intervention group will be administered BHT three times a day for four weeks and followed up for four more weeks after therapy completion. Patients in the waitlist-control group will undergo the same intervention and follow-up after a four-week waiting period. The primary outcome is change in the Korean version of the Hamilton Depression Rating Scale (K-HDRS) scores for major depressive disorders after four weeks. The secondary outcomes include scores on the K-HDRS, Korean Symptom Check List 95 (KSCL-95), Insomnia Severity Index (ISI), State-Trait Anxiety Inventory-Korean version (STAI-K), State-Trait Anger Expression Inventory-Korean version (STAXI-K), and the Instrument on Pattern Identifications for Depression and EuroQol-5 Dimension (EQ-5D).
Conclusions: This trial will provide high-quality clinical evidence of the efficacy and safety of BHT in the management of depression.

4The Effect of Alternative Medicine on Quality-of-Life Outcomes for Dementia Patients: A Systematic Review and Meta-Analysis

Objectives: As effective treatments for dementia are lacking in Western medicine, complementary and alternative medicine (CAM) is considered a useful option. While the quality of life (QoL) is a vital outcome for patients with dementia, the QoL of patients receiving CAM for dementia remains ambiguous. This study aimed to determine the effect of CAM on QoL outcomes in dementia patients.
Methods: A search was performed using the keywords “dementia,” “Alzheimer's,” “cognitive impairment,” “Chinese,” “Korean,” “oriental,” “herbal,” “acupuncture,” and “quality of life”. All quantitative data were synthesized using R version 4.1.1.
Results: Twenty-five randomized controlled trials (RCTs), 16 pre-post trials, and two cohort studies were selected for the systematic review. QoL in Alzheimer's disease (QOL-AD) (n=11, 25.6%) and geriatric QoL in dementia (GQOL-D, n=9, 20.9%) were the most utilized QoL instruments. Significant benefits in QoL were observed after receiving mind, body, combined mind and body, nursing, oriental medicine, and acupuncture therapies. In the meta-analysis, the combined effect was shown to significantly increase QOL-AD compared to before CAM interventions (standardized mean difference, SMD: 0.507; 95% confidence interval (CI), 0.191∼0.824; p<0.01). The overall synthesized estimates in the GQOL-D showed a significantly improved QoL (SMD: 0.537, 95% CI: 0.238∼0.837 p<0.01; one group; SMD: 1.465, 95% CI: 0.934∼1.996, p<0.01). The seven studies assessing the cost-effectiveness of CAM reported uncertain outcomes.
Conclusions: This study showed that CAM interventions benefited patients with dementia by improving their QoL. While additional standardized research is required, CAMs are suggested as effective clinical management for patients with dementia. They are also suggested as complementing therapies for these patients.

Objectives: To review trends of clinical trials on Korean medicine treatments for postoperative sleep improvement.
Methods: We searched randomized controlled trials (RCTs) on Korean medicine treatments for postoperative sleep improvement from ten domestic and foreign databases. Sample sizes, diseases, types of operation, diagnosis tools, pattern identification, interventions, outcome measurements, and main results of included studies were extracted and analyzed.
Results: A total of 20 RCTs were selected. Most studies were published in China. The most common target disease was cancer, followed by cardiovascular disease. Most studies lacked detailed description regarding participants such as onset, duration of sleep disturbance, and preoperative sleep issues. Herbal medicine was the most frequently used in 12 studies. The most commonly used prescription was Suanzaoren decoction. The effectiveness of Korean medicine treatment on improving postoperative sleep was found to be significant in most studies.
Conclusions: Korean medicine treatments might be effective in postoperative sleep improvement. However, the quality of included studies was low. Therefore, further well-designed research studies are needed to provide high quality clinical evidence on Korean medicine treatments for postoperative sleep improvement.

Objectives: The purpose of this clinical case study was to report the effect of Korean medicine treatment on a stroke patient with cognitive impairment, dysuria, and gait disturbance.
Methods: The patient was treated with Korean medicine, including herbal medicine, acupuncture, moxibustion, and cupping. We administered the Korean version of the Mini-Mental Status Examination (MMSE-K) Global Deterioration Scale (GDS) and Manual Muscle Test (MMT) and investigated the frequency of nocturia to evaluate the effectiveness of the treatment and improvement in symptoms.
Results: After Korean medicine treatment, the patient's symptoms were significantly improved. according to the MMSE-K, frequency of nocturia, and MMT results.
Conclusions: This clinical case study suggests that Korean medicine treatment, including Wuchasinkihwan- gagam could be an effective treatment for stroke patients with symptoms of cognitive impairment, dysuria, and gait disturbance.

Objectives: The purpose of this study is to introduce the progress of treatment and improve clinical use after the application of Mindfulness & Loving Beingness psychotherapy, for a patient with Hwa-Byung.
Methods: The patient was diagnosed with Hwa-Byung which arose as a result of their spouse's affair. The patient mainly reported chest pains, breathing difficulties, fear, and anxiety as symptoms. To address these, we administered a combination of M&L psychotherapy and Korean therapy including Herbal medicine, acupuncture, and moxibustion. Beck's Depression Inventory(BDI), Beck's Anxiety Inventory(BAI), and Hwa-Byung scale were evaluated before and after the treatment, to assess the clinical effect.
Results: Following treatment, the patient's overall clinical symptoms of chest pain, breathing discomfort, fear, and anxiety were significantly reduced. Additionally, their scores on the BDI, BAI, and Hwa-Byung scales decreased.
Conclusions: M&L psychotherapy and Korean therapy can be effective in treating Hwa-Byung.

Objectives: To report a case of a patient with a mild depressive episode accompanied by physical symptoms such as hyposmia visiting a Korean medicine hospital who was treated with oriental psychotherapy in parallel with existentialism-based psychotherapy showing improvement.
Methods: For this patient, Korean medicine Psychotherapy was mainly used for treatment. In parallel, existential Psychotherapy, acupuncture treatment, and self-relaxation training were implemented. For estimating this treatment, changes in results of Beck Depression Index (BDI), Beck Anxiety Index (BAI), Numeric Rating Scale (NRS), Korean Version of Sniffin' Sticks Test (KVSS), Butanol Threshold Test (BTT), Cross-Cultural Smell Identification Test (CC-SIT), and somatic symptoms that the patient subjectively expressed were determined.
Results: After the comprehensive treatment, it was confirmed that changes in BDI, BAI, KVSS, BTT, CC-SIT, NRS, and subjective somatic symptoms expressed by the patient all showed improvements.
Conclusions: We propose that a comprehensive treatment using Korean medicines Psychotherapy in parallel with existential psychotherapy can be used for patients who complain of physical symptoms accompanied by depression and hyposmia.

The purpose of this study is to report a case of a patient with pattern identification of liver fire flaming upward and metabolic syndrome improved by Korean medicine. The patient felt completely lost. She was treated for headache, insomnia, and metabolic syndrome with Korean medicine and psychotherapy. Pittsburgh Sleep Quality Index (PSQI), Insomnia Severity Index (ISI), Pattern Identification Tool for Insomnia (PIT-Insomnia), Minnesota multiphasic personality inventory-2 (MMPI-2), and bioelectrical impedance analysis (BIA) were used for assessment. During treatment for six months, the patient stopped taking headache pills. Her blood pressure was stabilized. Her body weight, body fat, body mass index (BMI), and visceral fat decreased significantly. The sum of ISI, PSQI, PIT-insomnia also decreased significantly. Her Ego strength increased in the MMPI-2 supplementary scale. We assume that Korean medicine treatment through acupuncture, herbal medicine, and psychotherapy is effective in treating pattern identification of liver fire flaming upward and metabolic syndrome.

Objectives: The purpose of this study was to investigate the experience and perception of using Korean medicine treatments for patients with mental disorders.
Methods: The method of a qualitative analysis was used, and semi-structured interviews were conducted. The researchers developed the survey questionnaires with consultation from psychiatrists from various university hospitals. Before conducting interviews, an IRB was approved KHSIRB-21-131 (RA), and the Hankook Research Company managed the interviews. The question composition comprised 11 questions based on symptoms and treatment situations accompanying treatment due to mental distress, perception of Korean medicine, awareness of treatment costs, and awareness of treatment costs.
Results: A total of six patients were interviewed. Each interviewee took 90 minutes to answer questions. Through the analysis of the data, the patients comprised four categories: characteristics, awareness of Korean medicine, awareness of Western treatment, and daily life due to mental disorder. The experience and perception of patients using Korean medicine showed positive attitudes. There was an opinion among patients that Korean medicine treatment was less dependent on drugs, and had fewer side effects, than Western medicine. Some patients said that Korean medical treatment is expensive and burdensome medical expenses. Some patients were not aware of the application of insurance to mitigate medical expenses.
Conclusions: Based on the findings of the study, it is urgent to expand the application of insurance benefits to Korean medicine.

Objectives: Brainspotting is a relatively new type of brain-body psychotherapeutic approach discovered and developed by David Grand. The objective of this study was to introduce possible clinical application of Brainspotting in Korean medicine.
Methods: The background, basic tools, standard processes, and principles of Brainspotting are presented mainly in reference to “Brainspotting: The revolutionary new therapy for rapid and effective change” published by David Grand.
Results: There are many similarities between Brainspotting and Korean medicine, such as Mind-Body holism, non-prejudiced attitude of therapist like Tao and wu-wei, and the importance of the eyes to the mind. They also share similarities in methods such as Iijungbyunqi and Qigong.
Conclusions: Brainspotting is expected to be applied to Korean medicine in various forms. It needs to be researched more in the future.

Objectives: The purpose of this study was to evaluate the quality of reporting for case reports published in Journal of Oriental Neuropsychiatry from year 2018 to 2021 compared with year 2013 to 2017 in order to recommend ways to improve the quality of case reports published in the future.
Methods: To evaluate the quality of case reports identified by electronic searching in Oriental medicine Advanced Searching Integrated System (OASIS) and by hand searching from archives on peer review system of Journal of Oriental Neuropsychiatry using CAse REport (CARE) guideline. The researcher assessed the quality of reporting based on the CARE guideline as 'Sufficient', 'Not-Sufficient', 'Not- Applicable', and 'Not-Reported' for case reports published from 2018 to 2021 in Journal of Oriental Neuropsychiatry. In addition, it was compared with previous case reports published from 2013 to 2017.
Results: Finally, 17 case reports were included for the assessment. General quality of reporting for case reports published from year 2018 to 2021 was improved compared with that of previous case reports published in 2018. The maximum value (46.4%→60.7%, 14.3% increase), the minimum value (22.2%→32.1%, 9.9% increase), and the median value (39.3%→50.0%, 10.7% increase) of the report rate evaluated as 'Sufficient' were generally improved. The maximum value (53.6%→50.0%, 3.6% decrease), minimum value (25.9%→21.4%, 4.5% decrease), and median value (35.7%→32.1%, 3.6% decrease) of the report rate evaluated as 'Not-Sufficient', the maximum value of the report rate evaluated as 'Not-reported' (40.7%→25.9%, 14.8% decrease), the minimum value (14.7%→10.7% decrease), and the intermediate value (14.7% decrease) were also generally improved. Four items (8b, 8d, 10c, and 13) were evaluated as 'Not-Reported'. These items seem to be items that need urgent improvement along with 8c, which showed a significant decrease in reporting rate.
Conclusions: There are needs to improve the quality of case reports in Journal of Oriental Neuropsychiatry by comparing case reports published from year 2013 to 2017 with case reports published from year 2018 to 2021. To improve the quality of case reporting, sufficient education at the academic level should be provided on thesis preparation methods. It is also necessary to develop a tool for evaluating the quality of case reporting that reflects characteristics of Korean Medicine.

Objectives: To perform a systematic review of placenta pharmacopuncture for treating neuropsychiatric diseases, focusing on its efficacy and the safety so that evidence on its clinical use could be obtained, thus contributing to further studies.
Methods: Through Korean, English, and Chinese databases (OASIS, Korean TK, KISS, RISS, ScienceON, Pubmed, Cochrane, EMBASE, CINAHL, AMED, CNKI, and Wanfang), combinations of keywords (placenta, pharmacopuncture, etc.) were used to select clinical studies published until January 2021 about placenta pharmacopuncture for neuropsychiatric diseases. Interventions included combined treatments. Study design included cases studies, series, and clinical trials. Cohort studies, literature reviews, in vitro and animal experiments were excluded. The primary outcomes involved measurements of symptoms, Visual Analogue Scale, or questionnaires. Data extracted from databases were imported to Endnote X7 to remove duplicates. The quality of the literature was assessed based on CAse REports Guidelines and Cochrane's Risk of Bias (ROB).
Results: Twenty-one studies were selected, including ten case reports, three case series, two onearmed clinical trials, one non-randomized clinical trial, and five randomized clinical trials. There were six studies on sleep disorders, five studies on stroke sequela, two on mood disorders, two on enuresis, two on Guillain-Barré syndrome, two on multiple sclerosis, one on neurocognitive disorder, and one on vertigo. The most frequent combined treatment was acupuncture in both the experimental group (n=10) and the control group (n=3). Acupoints were ST36, SP6, BL23, CV4, GB20, GV20, N-HN54, and so on. All studies reported improvement of symptoms. The quality of case studies was relatively high. Assessment of ROBs resulted in low risks.
Conclusions: Placenta pharmacopuncture is effective for neuropsychiatric diseases such as sleep disorders, mood disorders, enuresis, and neurocognitive disorders. Regarding insomnia, several studies have reported significant improvements with placenta pharmacopuncture. There was no adverse event associated with placenta pharmacopuncture.

5Herbal Medicine for Pediatric Epilepsy: Clinical Research Trends in Traditional Chinese Medicine

Pediatric epilepsy, a chronic, recurrent brain disorder, is the most common neurological disorder in children. Its prevalence is increasing. Early management is very important since 30∼40% of cases persist into adulthood. To provide basic data for future clinical research on pediatric epilepsy using Korean medicine treatment and cooperation between Western medicine doctors and Korean medicine doctors, we reviewed recent clinical research in traditional Chinese medicine (TCM) using herbal medicine for pediatric epilepsy. A total of 23 articles (1 clinical practice guideline, 3 systematic reviews, 15 randomized controlled trials (RCTs), and 4 non-RCTs) were reviewed in this study. The authors summarized characteristics of included studies regarding study subjects, diagnostic tools, pattern identification tools, treatment period, evaluation tools, detail of herbal medicines, treatment effects, and adverse events. Combination therapy using both herbal medicine (HM) and anti-epileptic drugs (AEDs) was performed more frequently than herbal medicine alone. Liver-pacifying medicinal, water-draining medicine, and orifice-opening medicine were frequently used. The main single HMs were Cheonma, Boglyeong, Jogudeung, and Seogchangpo. Combined therapy using HM and AEDs had significant benefits in improving total effective rate. It also appeared to be safer than AEDs. However, since the quality of clinical trials was poor and only studies in the last 10 years were included, the clinical evidence was uncertain. Finally, the authors provided limitations of this study and several suggestions for future research based on our analysis results.

Objectives: The purpose of this study was to report the effect of Korean medicine (KM) treatment on a patient diagnosed with cluster headache.
Methods: The patient included in this study was diagnosed with cluster headache who received KM therapy (herbal medicine, acupuncture, moxibustion, cupping, and chu-na treatment). The main symptom of the patient was pain in the left occipital region and orbital region. It was diagnosed as a pattern of liver depression and spleen deficiency by oriental medicine. During the hospitalization period, treatment effect was evaluated by NRS. HIT-6. BDI, and STAI were additionally performed to assess depression and anxiety.
Results: After 24 days of treatment, the intensity of headache decreased from NRS7 to NRS3. During follow-up at two weeks, there was no pain. Normal daily activities were possible.
Conclusions: Korean medical approach might be useful for pain relief and restoration of daily living ability for patients with cluster headache.
