Data was collected for the period 2002 and 2013, from The National Health Insurance Service Health-Examinee Cohort Database, to evaluate the association between periodontal disease (PD) and vasculogenic erectile dysfunction (VED). Univariate and multivariate logistic regression models applied to a longitudinal retrospective database assessed the association between PD and VED, while adjusting for confounders of socio-demographic factors (age, household income, insurance status, health status, residence area, and smoking status) and systemic disease (cerebral infarction, myocardial infarction, angina pectoris, hypertension, diabetes mellitus, osteoporosis, lipid metabolism disturbances, and obesity). The univariate analysis revealed that VED was positively and significantly related to PD (OR = 2.07; 95% CI 1.73-2.49; P < 0.001). This was confirmed in the multivariate analysis, after adjusting for socio-demographic and systemic disease (OR = 1.45; 95% CI 1.21-1.75; P < 0.001). Using information from a longitudinal retrospective cohort database and considering the limitations of this study, we found positive and significant associations between PD and VED. Further studies are required to assess the mechanism underlying the association between PD and VED.

Certain pathogenic bacteria obtain iron for growth through heme or heme compounds. Streptococci are gram-positive bacteria that reside in the mouth and require iron for growth. However, the iron accumulation mechanism in streptococci remains unknown. Therefore, this study investigated the hemin-binding properties of streptococci to confirm the potential of oral streptococci to obtain iron through heme compounds. The hemin-binding ability of streptococci was evaluated by incubation with hemin and subsequent measurement of the optical density of the supernatant to determine the amount of hemin bound by the bacteria using the hemin concentration standard curve. The hemin-binding of streptococci was proportional to the concentration of bacteria (Streptococcus gordonii) and hemin added. However, when a high hemin concentration of up to 60 μg/mL was added, the amount of hemin bound by S. gordonii remained almost unchanged. The incubation temperature had no significant effect on hemin-binding, but pretreatment with trypsin and protease decreased hemin-binding. Additionally, we experimentally confirmed that S. gordonii could use hemin for growth. This study demonstrated that streptococci possess hemin-binding sites and can acquire iron through hemin.

Various disinfectants are used to remove biofilms formed in dental unit waterlines (DUWLs). This study aimed to assess the antibiofilm effects of cetylpyridinium chloride (CPC) on DUWL biofilms formed in a CDC Biofilm Reactor model and evaluate the feasibility of CPC as a novel DUWL disinfectant. For experiments, CPC was used at concentrations of 0.003125% to 0.00039%. Well-established DUWL disinfectants, such as 0.5% sodium hypochlorite (NaOCl), 0.12% chlorhexidine (CHX), and a mixture of 0.12% CHX and 12% ethanol, were used as standards. DUWL biofilms formed in the CDC Biofilm Reactor model were exposed to the disinfecting agents for 15 min. The number of living bacteria in the biofilms before and after exposure to chemicals were compared, and the biofilm reduction rate was calculated. The biofilm reduction rate of CPC at a concentration higher than 0.001562% was greater than that of all other conventional disinfecting agents. These findings suggest that CPC can be used for effective removal of DUWL biofilms.

3Evaluation of antioxidant and anticancer activities of fermented pericarp extract of Camellia japonica L. using Aspergillus oryzae in oral cancer

This study aimed to investigate the antioxidant and anticancer activities of the fermented extract of the dried pericarp of Camellia japonica L. (CJ). The antioxidant activity of the extract after fermentation by Aspergillus oryzae (A. oryzae) at different doses, times, and temperatures was slightly decreased compared with that of the unfermented extract. The total polyphenol content decreased significantly as fermentation time was increased at 30℃, but there was no significant change when the fermentation was performed for 72 hr at different temperatures. The total flavonoid content was not significantly changed by fermentation conditions. Evaluation of the anticancer activity of the A. oryzae-fermented extract revealed that there was no effect on the viability of immortalized 293T cells treated with the extract for 48 hr, but the viability of YD-8 and YD-10B oral cancer cells was significantly reduced. In addition, there was a significant increase in the expression of cleaved PARP and caspase 3, and in apoptotic (annexin V/PI-stained) cells. These results suggest that the water extract of dried pericarp of CJ has no anticancer activity, but gained anticancer activity following fermentation by A. oryzae; furthermore, the fermented extract may be a potential treatment for oral cancer.

This study aimed to compare the vertical and horizontal bone augmentation achieved when using different types of titanium mesh (Ti-mesh) on edentulous atrophic maxilla. Surgical intervention was performed in the maxillary molar area of three patients. One patient received A type Ti-mesh (aTM; pore size: 0.9 mm, thickness: 0.1 mm), and placement of one implant was carried out after 6 months. The remaining two patients received B type Ti-mesh (bTM; pore size: 1.7 mm, 3.37 mm; thickness: 0.3 mm;), and 4 implants were placed after 6 months. The findings showed that both types of Ti-mesh were associated with good feasibility and predictability with regard to vertical and horizontal bone augmentation. Pore size did not affect the amount of bone regenerated for implant placement. However, bTM was associated with several advantages, including decreased operation time and increased convenience during guided bone regeneration surgery.

Pulpotomies are usually the treatment of choice for vital primary teeth where the removal of deep caries may lead to pulp exposure. These procedures entail removal of the inflamed coronal pulp, application of a pulp capping material, and preservation of the vitality of the radicular pulp. Inaccurate diagnosis or defective restorations may lead to failure of pulpotomies and consequent apical inflammation and cyst formation. The spread of apical inflammatory lesions affecting deciduous canines to the periodontal tissues can cause displacement and impaction of the permanent successor tooth. As this is a serious side effect of pulpotomies, precise diagnosis and long-term follow-up are required before and after the treatment. This case study presents three patients with canine impaction caused by unsuccessful pulpotomy of the deciduous canine.

This case report presents the 8-year follow-up result of autotransplantation on a healed alveolar ridge, with sinus elevation and autogenous bone grafting. We present a case of a 30-year-old male patient whose maxillary first molar was extracted 1 year ago. Under general anesthesia, after the membrane of the left maxillary sinus was elevated, an artificial socket was prepared on the healed alveolar ridge of the left maxillary first molar area. The left maxillary third molar was extracted and tried on the prepared artificial socket. The autogenous bone was harvested and grafted to the left maxillary sinus floor. The transplanted tooth received root canal therapy 2 months after surgery. During the 8-year follow-up period, no discomforts, periodontal ligament (PDL) widening, or root resorption signs were observed. Desirable bone remodeling signs and regeneration of PDL were observed on the radiograph. Therefore, autotransplantation can be a treatment option for healed and deficient alveolar ridge.

Continuous alcohol ingestion may lead to various dental diseases because a hyposalivation. In this study, we investigated changes in pathology and choline acetyltransferase (ChAT, an enzyme for synthesis of acetylcholine) in the brain and acetylcholine-binding protein (AchBP, a protein responsible for modulation of synaptic transmission in cholinergic synapses) in the submandibular gland after alcohol ingestion. There was no change in neuronal death and expression of ChAT in superior salivary nucleus after alcohol ingestion. In the submandibular gland, expression of AchBP were significantly reduced after alcohol ingestion. Therefore, decrease of saliva secretion by alcohol ingestion is less related to damage of neurons in the superior salivary nucleus and decrease of acetylcholine synthesis or cholinergic neurotransmission.

Most root canal procedures are anatomically complex, resulting in insufficient removal of the necrotic pulp by mechanical root canal shaping. This study evaluated the use of antibiotics as canal irrigants for removal of susceptible intracanal bacteria, using an Enterococcus faecalis biofilm model. Enterococcus faecalis biofilm was developed in the root canal and test tube. For the experiments, sodium hypochlorite (NaOCl), augmentin and erythromycin were used as intracanal irrigants. The test tubes and canals were prepared and irrigated with phosphate buffered saline (PBS), NaOCl, augmentin and erythromycin. Bacterial samples were collected after irrigation in the test tube models on day 1, and from the tooth model on days 1, 4 and 7. The surface of each sample and attached pattern of the bacteria was also analyzed by examining under scanning electron microscopy (SEM). The antibacterial study using 10 ml test tubes (n=10) revealed that 5% NaOCl, augmentin and erythromycin inhibited bacterial growth relative to PBS. In the tooth model, NaOCl, augmentin and erythromycin inhibited bacterial growth significantly (all p<0.05) at days 4 and 7, relative to PBS. Compared to day 1, bacterial density of all groups reduced at day 7, and changes in cell morphology were observed in all experimental groups. Our studies revealed evidence of significant differences in the antimicrobial efficacy at days 4 and 7, upon irrigation with augmentin and erythromycin versus PBS, in root canals infected with E. faecalis.

Various medical policies for elderly patients have been introduced in the era of aging. Recent implementation of nursing home dentist systems is a positive opportunity to recognize the significance of oral health care in managing general healthcare and improving the quality of life for elderly people. In addition, it is necessary to strengthen dentists` competencies and key roles in recovering oral function and rehabilitation with changes in the customer-oriented dental care paradigm. This study suggests the path towards stabilizing and activating nursing home dentist systems by understanding the background and current status of nursing home dentist systems, and identifying ways of improving the systems.

There have been few reports about the manpower planning of dentists in the public sector in Korea. The purpose of this study was to research dentists` opinions about the public posts, and to suggest public health policies for the new dental manpower position. A total of 536 dentists were asked to fill out questionnaires inquiring about their opinions of public posts. We used an online survey methodology to involve 500 participants. The chi-square tests and frequency distributions were implemented to analyze the dentists` opinions about the public posts. The significance level of 0.05 was applied. A statistical package SPSS 21.0 was used for the analysis. An overwhelming majority of dentists (95.6%) answered that it is necessary to set up public posts for dentists. Respondents pointed out that the Korean Dental Association (35.5%) and the Ministry of Health and Welfare (33.2%) were the main institutes that should carry out this process. Especially, 47.4% of dentists answered they would quit their job if they had opportunities to fill public posts. Most dentists wanted to work as administrative officials (36.3%) in the public sector. Many dentists (62.4%) wanted to contribute to public welfare as health care professionals in public posts. Most of all, respondents (95.6%) answered that it is imperative to improve the manpower system of dentists in public posts.

Currently, outcomes of dental stem cells studies indicate that the tooth is an available source for clinical application. Finding a universal cell that can morph into many several types of oral tissues is a key for new tooth creation. If dental stem cells are isolated to form new tooth buds, with creation of an imitative condition to the embryonic jaw, this will be a revolutionary new way to replace a missing tooth. Although investigators have created tooth-like structures in studies, so far, later dental stem cells investigations will be able to create a workable vascularized implant in the human body. This review study discusses current challenges in creating a biological environment that mimics the condition of teeth as they form in an embryo.

Tooth autotransplantation is the transplantation of embedded, impacted, or erupted teeth from one site to surgically prepared sockets or extraction sites within the same individual. Tooth autotransplantation is a valuable method to replace missing teeth, instead of other procedures like dental implants or prosthetic replacements. Discrimination of indication and consideration of delicate factors affecting the results is important to success in the procedure. This study reports a 5 year clinical observation of two different cases.

Pre-eruptive intracoronal resorption is a rare disease defined as an intracoronal radiolucent lesion. Most are revealed through casual radiological examinations, and are clinically identifiable after eruption. The possible causes of lesions include congenital abnormalities, systemic fluoride uptake, external factors after tooth formation, and eruption of the tooth; however, no definite etiology has been found. Early diagnosis through regular examination is therefore considered to be the best diagnostic method. Treatment may differ depending on the size and progression of the lesion, but conservative treatment is recommended. This case report describes two cases where the lesions were accidentally found while performing a radiologic examination addressing other issues. It is the clinician`s responsibility to identify the size of the lesion and the expected eruption time, and to execute the appropriate treatment.

Orthodontic mini-implant is usually placed in the buccal interdental area for molar distalization or total arch distalization. However, this requires timely replacement of the mini-implant to prevent contact between root and mini-implant and ensuing root resorption. Alternatively, midpalate may be considered as the mini-implant placement site because it has considerably dense bone and there is no interference with roots. Also, distalization forces may be designed to pass through the center of resistance of dentition for translatory distal movement. In this case report, end-on Class II occlusion with lip protrusion was successfully treated by applying distal forces from a transpalatal arch to a lever attached to two midpalatal mini-implants. Anti-rotation bends were bent on transpalatal arch to prevent mesial rotation of first molars during distalization. Another benefit of this approach is that molar distalization may be initiated at the beginning of treatment because distal tipping of molars can be prevented by applying distal force at the level of the center of resistance of molars thus eliminating the need of stiff rectangular archwire. With this approach, time-consuming molar distalization may be initiated at the beginning of treatment and therefore, overall treatment duration can be reduced.

Numb chin syndrome (NCS) is sensory neuropathy characterized by numbness in the chin and lower lip, involving distribution of the mental nerve. Causes of NCS may be of regional origin, a systemic disease, and malignancy. In this study, we report a case of NCS occurring as a first symptom of malignant metastasis in a 57-year-old female patient diagnosed with hyperparathyroidism.
