Purpose: This study was conducted to assess the effects of robot-assisted gait training with visual feedback on gait, balance, and balance confidence in patients with chronic stroke. Methods: Thirty subjects with chronic stroke were randomly assigned to two groups: the experimental group (n=15) and the control group (n=15). The experimental group performed robot-assisted gait training for 30 minutes and the control group performed gait training with assisted devices training for 30 minutes after both groups performed conventional physical therapy for 30 minutes. Both groups performed the therapeutic interventions for 5 days per week, for a period of 4 weeks. For assessment of the 10 m walking test (10 MWT), Figure of 8 on the walk test (F8WT), Timed-Up and Go test (TUG), and Berg Balance Scale (BBS) were used to test the gait and balance, and the Korean version of the Activities-specific Balance Confidence Scale was used to test the balance confidence. Results: The experimental group showed significant improvement in the 10 MWT and the K-ABC (p<0.05), and the control group showed significant improvement in the BBS and the TUG (p<0.05). In four measurements, there were significant differences between the two groups (p<0.05), and the control group showed significant improvement in the F8WT at pre and post intervention (p<0.05). Conclusion: This study demonstrated that Robot-assisted gait training with visual feedback is an effective intervention for improving straight gait abilities and balance confidence, while the control group showed some improvement in curve gait and balance. Thus, we suggest both Robot-assisted gait training with visual feedback and gait training with assisted devices training exercise as a therapeutic intervention in chronic stroke rehabilitation.

1Effects of EMG-Biofeedback Using Closed Kinetic Chain Exercise on Q-angle and Quadriceps Muscle Activation in Patellofemoral Pain Syndrome

Purpose: The aim of this study was to determine the effects of electromyographic (EMG)-Biofeedback using closed kinetic chain exercise (EB-CKCE) on quadriceps angle (Q-angle) and quadriceps muscle activation and muscle activation ratio in subjects with patellofemoral pain syndrome and to provide fundamental information on rehabilitation exercise in patellofemoral pain syndrome. Methods: Thirty participants who met the criteria were included. The subjects were randomly divided into three groups: control group (Group I, n=10), semi-squat exercise group (Group II, n=10), and EMG-Biofeedback using closed kinetic chain exercise group (Group III, n=10). Intervention was provided to each group for eight weeks (three times per week; 30 minutes per day). Subjects were measured on Q-angle and quadriceps muscle activation. Results: Significant difference in Q-angle and quadriceps muscle activation was observed in groups II and III compared with control group I (p<0.01). Results of post-hoc analysis showed a significant difference in Q-angle and quadriceps muscle activation in on group III compared with groups I and II. Conclusion: Findings of this study suggest that closed kinetic chain exercise using EMG-Biofeedback that provides real-time biofeedback information on muscle contraction may have a beneficial effect on improvement of Q-angle and quadriceps muscle activation in patellofemoral pain syndrome.

Purpose: The purpose of the study was to review functioning components of studies for measuring functional information using International Classification of Functioning, disability and health (ICF) in Korea. Methods: A literature review was conducted and eligible studies were identified via search of RISS, KISS, DBpia, KoreaMed, PubMed, and ISI databases published from 2001 to 2015. For inclusion in this review, a study should be published in a peer-review journal, must have participation of Korean researchers and/or developed in Korea, and should provide functioning data related ICF. Descriptive studies containing only ICF concepts and review studies were also excluded. Collected functioning data in each study were analyzed using frequency based on ICF domain. Data assessment was performed by two independent reviewers. Results: Eighty publications were included in the analysis. The majority of studies collected data from clinical patients (n=38) and from the disabled (n=28). Fourteen studies assessed functioning data from the elderly and students under special education. The studies reported functioning data using various tools, including ICF itself, ICF checklist, coresets, and conventional measurement tools. Body function domain was most commonly used and the least used domain was the body structure across areas. Interestingly, increasing data related to environmental factors was observed in all studied populations. Conclusion: Functioning data was collected from four domains of ICF across professional areas. The most common collected data was body function and activity and participation for which conventional measurement tools are already available. To understand clients, components of environmental factors that might influence a person`s functioning should be considered.

4A Comparison of Shoulder Stabilizer Muscle Activities of Therapeutic Climbing and Isometric Exercise in Patients with Shoulder Impingement Syndrome

Purpose: The purpose of this study was to compare therapeutic climbing exercise and general isometric exercise in patients with shoulder impingement syndrome. Methods: Among 20 adults, study subjects were arbitrarily classified into an experimental group of 10 and a control group of 10. The control group performed general isometric exercise (ISE) and the experimental group performed therapeutic climbing exercise (TC) (3 sets, 3 times per week for 8 weeks). To evaluate the effects of exercise, subjects were evaluated using a Disabilities of the arm, shoulder and hand score (DASH), a goniometer for range of motion, and shoulder activity measured serratus anterior, upper trapezius, and lower trapezius. Independent and paired t-test were used for comparison of the effect between groups. Results: DASH scores showed a significant decrease in both groups after 8 weeks of treatment (p<0.001) and significant difference was observed between the TC groups (p<0.01). Flexion and abduction were significantly increased after 8 weeks of treatment in the ISE group (p<0.001) and flexion, abduction, external and internal rotation were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Serratus anterior and lower trapezius activity were significantly increased after 8 weeks of treatment (p< 0.001) and upper trapezius activity was not significantly increased after 8 weeks of treatment in the ISE group (p >0.05). Serratus anterior, lower trapezius, and upper trapezius activity were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Conclusion: Scapular stabilizing exercise using a therapeutic climbing exercise increases range of motion and decreases DASH, and increases activity of shoulder muscles in patients with impingement syndrome.

5Comparison of Muscle Activities of Trunk and Lower Limb during Bow and Squat Exercises

Purpose: The purposes of this study were to examine muscle activities of trunk and lower limb during squat and 108 bows exercises and to provide objective data for establishing a training method for improving muscle strength of trunk and lower limb. Methods: Twenty normal healthy subjects participated in this study. Each exercise was divided into five periods. Muscle activities of trunk and lower limb in each period of both 108 bows and squat exercises were measured and analyzed by independent t-test. Results: In starting, mid-flexion, mid-extension, and end period muscle activities obtained from 108 bows exercise were significantly higher than those from squat exercise. However, in the final flexion period, muscle activities of multifidus, elector spinae, rectus femoris, biceps femoris, and tibialis anterior from squat exercise were significantly higher than those from bow exercise. Conclusion: In this study, high muscle activities in most muscles of trunk and lower limb were observed from all periods of 108 bows exercise except the final flexion period. Therefore, it is likely that 108 bows exercise rather than squat exercise is more suitable for high strength exercise to improve muscle strength of trunk and lower limb and thus will be applicable for strengthening muscles of trunk and lower limb of patients.

6The Effects of Group Exercise, Manual Therapy and Home Exercise on Pain, Range of Motion and Function in Patient with Adhesive Capsulitis

Purpose: The purpose of this study was to determine the effects of group exercise, manual therapy, and a home exercise program on VAS (visual analogue scale), ROM (range of motion) of shoulder, and functional outcome in adhesive capsulitis. Methods: The study was conducted with 45 patients. All subjects were randomly assigned to 3 groups: group exercise (n=15), manual therapy (n=15), home exercise (n=15). The subjects performed an intervention program 3 times a week for 4 weeks a total of 12 times. Outcome measurements were VAS, ROM, and Constant-Murley score outcome measures. Measurements were taken four times, at baseline, 4 weeks later, 8 weeks later, and 12 weeks later. Results: VAS, ROM of the shoulder, and functional outcomes showed improvement in all groups at the final follow-up (p< 0.05). VAS and ROM showed greater improvement with group exercise and manual therapy than home exercise, however, there were no significant differences between the two groups (p >0.05). Functional outcomes showed the greatest improvement in group exercise at the final follow-up. This improvement was greater than with manual therapy or home exercise (p<0.05). Conclusion: Group exercise and manual therapy were more effective for improving pain, ROM, and function than home exercise in adhesive capsulitis. In particular, group exercise is more effective in functional recovery than other physiotherapy interventions.

7Effect of Glenohumeral Joint Rotation on Serratus Anterior, Pectoralis Major and Upper Trapezius EMG Activity during Push-up Plus Exercise

Purpose: The aim of this study is to determine the effect of glenohumeral (GH) rotation position in modified knee push-up plus exercise (MKPUP) by examining the surface electromyography (EMG) amplitude in serratus anterior (SA), pectoralis major (PM), and upper trapezius (UTz) and the activity ratio of each muscle. Methods: A total of 22 healthy subjects volunteered for the study. Each subject performed the MKPUP at 0°, 45°, and 90° of GH joint internal rotation. EMG of the SA and PM, UTz was compared between GH rotation positions and each muscle activity ratio. EMG was used to measure the muscle activity in terms of ratios to maximal voluntary isometric contraction (MVIC). Results: The difference in EMG activity during the exercise in three GH joint internal rotation positions was observed with the SA and the PM. The greater the GH joint internal rotation angle was, the lower the activity of the PM. In contrast, the SA showed higher activity. However, the activity of UT was similar under all conditions. The ratio of the SA and the PM was considerably greater at 90° GH joint internal rotation than at 0° and 45°. Conclusion: When excessive activation of the PM or imbalanced activation between the PM and the SA occurs, the MKPUP exercise is most effective at 90° of GH joint internal rotation. Use of this position would be a beneficial strategy for selective strengthening of the SA and minimizing PM activation.

8Influence of Difficulty Variation of the Core Stabilization Exercise on Thickness Changes of Abdominal Muscles in Healthy Subjects: A Pilot Study

Purpose: This study examined thickness changes in abdominal muscles according to difficulty level of core stabilization exercise in healthy subjects. Methods: Thirty healthy subjects (age range: 21-30 years) volunteered under three conditions. In the first condition, the subjects performed an abdominal draw-in maneuver (ADIM). In the second condition, they performed the ADIM during quadruped exercise using a suspension device without extending their lower limbs. In the third condition, the subjects performed the ADIM during quadruped exercise using a suspension device while extending both lower limbs. The changes in thickness of transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles were measured by ultrasonography (US) imaging during the three experimental conditions, and US was used to measure the improvement ratio of muscle thickness at rest. The interventions were conducted over three trials in each condition, and measurements were performed on each subject by one examiner. Results: Our results showed a significantly greater increase in the muscle thickness of TrA and IO muscles after performance of quadruped exercise using a suspension device without knee extension (p<0.05) compared to the other conditions. The results also showed a significantly greater increase in the thickness changes of EO muscle in those who performed the ADIM during quadruped exercise using a suspension device with knee extension (p<0.05) compared with the ADIM only. Conclusion: These findings demonstrated positive evidence that a low-level core stabilization exercise could improve thickness of abdominal muscles.

9Reliability of Thickness Measurements of the Abductor Hallucis Muscle Using the Spring Gauge Technique in Hallux Valgus Subjects: An Ultrasonographic Study

Purpose: The purpose of this study was to determine the intra- and inter-rater reliability of muscle thickness (MT) measurements of the abductor hallucis (AbdH) in subjects with hallux valgus (HV), using ultrasonography performed at different inward pressures of approximately 0.5 kg, 1.0 kg, 1.5 kg, and 2.0 kg, with no pressure control. Methods: Thirty-two subjects with HV were recruited. The thicknesses of both sides of the AbdH were measured randomly by two different examiners for assessment of the intra- and inter-rater reliability. The measurement values were analyzed using the intra-class correlation coefficient (ICC) with a 95% confidence interval (CI). ICC (2,1) was used to determine the inter-rater reliability of MT measurements of the AbdH, while ICC (3,1) was used to assess the intra-rater reliability. Results: The results showed higher ICC values for intra-rater reliability compared to inter-rater reliability, and the value for inter-rater reliability with no pressure control (ICC=0.74 [95%CI=0.53-0.87]) was smaller compared to pressures of 0.5 kg, 1.0 kg, 1.5 kg, and 2.0 kg. Other inward pressures for intra- and inter-rater reliability also showed excellent values (ICC=0.86-0.96). Conclusion: The findings showed that maintaining consistent inward pressure is essential for reliable results in measurement of the MT of the AbdH by different examiners in a clinical setting.

10The Reliability of Flexor Hallucis Longus Stretch Test in Subjects with Asymmetric Hallux Valgus Angles

Purpose: The flexor hallucis longus stretch test can determine the shortness of the flexor hallucis longus muscle by measuring the angle of extension in the first metatarsophalangeal (MTP) joint at maximum ankle dorsiflexion. Less than 30 degrees of the first MTP joint at the maximal ankle dorsiflexion indicates shortness of the flexor hallucis longus muscle. The purpose of this study was to examine the intra- and inter-reliability of the flexor hallucis longus stretch test in subjects with asymmetric hallux valgus (HV) angles. Methods: Sixteen subjects with asymmetric HV angles participated in this study. In sitting position, dorsiflexion angles of the first MTP joint were measured with maximum ankle dorsiflexion on each side. ICC (3,1) and ICC (3,k) models were used, respectively, to assess the intra-reliability and inter-reliability of the flexor hallucis longus stretch test. The paired-t test was used to compare the dorsiflexion angle of the first MTP joint on the side with the smaller HV angle with that of the side with the larger HV angle. Results: The results of the study showed that both intra- and inter-reliability were more than 0.95 of the coefficient. Dorsiflexion angle of the first MTP joint was higher on the side with the smaller HV angle. Conclusion: Use of the flexor hallucis longus stretch test is acceptable in clinical settings because both intra- and inter-reliability were high in subjects with asymmetric HV angles. In addition, shortness of the flexor hallucis longus muscle is associated with HV angle. This study provides useful information for use in management of HV deformity.
