1식도암 수술 환자의 회복을 위한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석

Purpose:This study aimed to investigate the effects of non-pharmacological interventions in esophageal cancer surgery patients via a systematic review and meta-analysis. Methods: This study was conducted in April 2020 to update the original review. A systematic literature review was conducted based on the PubMed, EMBASE, CINAHL, CENTRAL, NDSL, RISS, KISS, and KMbase databases. Comprehensive Meta-Analysis (version 3.0) software was used to analyze the standard effect size, subgroup variables, and publication bias. Results: In total, 10 of 17 studies were used to estimate the effect size of non-pharmacological interventions such as education, exercise, management (temperature control, pain control, etc.), and nutritional support. These non-pharmacological interventions were effective in improving the quality of life (g, 0.15; 95% confidence interval, 0.10~0.21). However, pneumonia and lung function remained unaffected. The subgroup analysis according to the intervention type showed that education was effective in improving the quality of life, and interventions comprising education, management, and nutritional support were effective in reducing the incidence of pneumonia. Conclusion: In order to improve the quality of life and to prevent pneumonia in esophageal cancer surgery patients, non-pharmaceutical interventions comprising education, exercise, management, and nutritional support should be developed and provided.

Purpose: This study aimed to evaluate the effects of global health competency education on cultural competence, empathy, and cultural acceptance among nursing students in Korea. Methods: A literature review was performed until July 2021 in domestic electronic databases, including KoreaMed, RISS, KISS, and Google Scholar. Nine references that met the inclusion and exclusion criteria were selected and Meta-analysis was performed using Cochrane's risk of bias tool and the R version 3.5.2 (Meta-analysis with R) program. Results: The findings in this study indicate that effect size of global health competency education for cultural competence was Hedges'g=1.33 (95% confidence interval, 0.78~1.89) and the effect size of empathy was Hedges'g=0.82 (95% confidence interval, 0.07~1.57), as indicated by a large effect size. The effect size of cultural acceptance was Hedges'g=0.49 (95% confidence interval, 0.06~0.92), as indicated by a moderate effect size. The differences were statistically significant. In the meta-subgroup analyzed by approach intervention type, intervention time had a significant effect. The risk of publication bias is low. This study found that the global health competency education program for nursing students had an effect on cultural competence, empathy, and cultural acceptance. Conclusion: Various global health competency programs should be developed and implemented for nursing students to improve their multicultural health competencies.

3Influence of Late School-age Children and Their Mothers' Health Beliefs, Children's Self-esteem, and Social Support on Health Behaviors of Children

Purpose: This study aimed to investigate the predictors of late school-age children's health behaviors in children and their mothers' health beliefs, children's self-esteem, and social support. Methods: A descriptive correlational study was conducted with 152 late school-age children from the 4th to 6th grades and their mothers in Korea. Data were collected from February 19 to March 5, 2020. The collected data were analyzed using t-test, ANOVA, Pearson's correlation coefficient, and hierarchical multiple linear regression. Results: The average score for health behaviors was 3.35 out of 4.00. The highest subscale of health behavior was mental health, whereas the lowest was eating habits. After adjusting for the influences of the general characteristics of children and their mothers, children's perceived benefits (β=.17, p=.005), perceived barriers (β=-.20, p=.001), self-efficacy (β=.19, p=.005), and mothers' perceived severity (β=.18, p=.001) were significant predictors of health behaviors among late school-age children, accounting for 24.0% of the variance (R2 change=.24, F=11.59, p<.001). The total explanatory power of the final regression model was 72.0% (adjusted R2=.72, F=24.98, p<.001). Conclusion: Children's health beliefs were more important than mothers' health beliefs in explaining the health behaviors of late school-age children. Nursing interventions should focus on perceived barriers, self-efficacy, and perceived benefits to improve health behaviors of late school-age children.

Purpose: This study aimed to identify the effects of social contact and participation in social activities on the health-related quality of life (HRQoL) of the community-dwelling older adults who experience subjective cognitive decline. Methods: This cross-sectional study was conducted using data from the 2019 Community Health Survey. A total of 16,828 were included in the final analysis. Data were analyzed using unweighted frequency, weighted percentage, and logistic regression with IBM SPSS Statistics version 25.0. Results: In the analysis using logistic regression, social contact and participation in social activities were associated with almost all areas of HRQoL. Social contact and participation in social activities had the most significant influence on self-care (Odds Ratio [OR]: 0.75, Confidence Interval [CI]=0.72~0.79 / OR: 0.81, CI=0.76~0.85). Conclusion: Social contact and participation in social activities in community-dwelling older adults who experience subjective cognitive decline significantly impacted their HRQoL. These findings could suggest the existence of an effective approach for developing interventions to improve HRQoL.

Purpose: This study aimed to identify the types and causative factors of non-value-added nursing activities in outpatient departments. Methods: The participants were nurses working in the outpatient department of university hospitals located in two metropolitan cities. A total of 164 questionnaires were used for the final analysis. The research tool was a questionnaire composed of general characteristics, work-related characteristics, types of non-value-added nursing activities, and causes of non-value-added nursing activities in the outpatient department. These data were analyzed by frequency, percentage, and independent t-test and one-way ANOVA using SPSS 21.0. Results: The types of non-value-added nursing activities showed 'repeating' the most, followed by 'reverse-proxy working' and 'waiting'. Answering phone calls for reservation confirmation or replying to irrelevant inquiries, providing guidance to hospital facilities or directing patients to the location of other departments, and providing information to not seeing patients was occurred always or frequently. The causes of non-value-added nursing activities were clients ignoring regulations, problems with computerized systems, and the absence of patients from designated locations. In terms of career in the current department, there was a statistically significant difference in the categories of 'searching' (t=-2.44, p=.016) and 'reworking' (t=-2.23, p=.027) among non-valueadded nursing activities. There was a statistically significant difference between the number of aid nurses and 'reverse-proxy working' (t=-2.59, p=.010). There was a statistically significant difference between the number of patients and 'repeating' (F=3.39, p=.036), 'reverse-proxy working' (F=3.31, p=.039), and 'duplicating' (F=5.74, p=.004). Conclusion: This study is expected to be used as basic data to reduce non-value-added nursing activities and improve the efficiency of nursing activities in outpatient departments.

6Factors Affecting Job Satisfaction and Turnover Intention among Newly Graduated Nurses in Korea

Purpose: This study explored the factors affecting job satisfaction and turnover intention among newly graduated nurses in South Korea. Methods: A sample of 104 nurses who had worked less than 12 months in a National University Hospital were included. Self-report questionnaires that measured job satisfaction, turnover intention, perceived preceptor's coaching leadership, and self-leadership, were used to collect data. Collected data were analyzed using t-test, ANOVA, and quantile regression with Stata version 16. Results: Self-leadership was significant at all percentiles, and the possibility to request days off was a significant factor affecting job satisfaction in the 10th percentile (B=6.98, p=.005), 50th percentile (B=3.82, p<.001), 75th percentile (B=3.57, p=.013), and 90th percentile (B=6.20, p=.033). Moreover, the average number of night shifts per month was a significant factor affecting job satisfaction in the 10th percentile (B=2.76, p=.029). The possibility to request days off was a factor affecting turnover intention in the 25th percentile (B=-3.48, p=.010) and 50th percentile (B=-1.50, p=.037), and the average number of night shifts per month was a significant factor affecting turnover intention in the 90th percentile (B=-2.11, p=.037). Conclusion: To improve job satisfaction and reduce the turnover intention of newly graduated nurses, the nursing organization needs policies that reinforce nurses' self-leadership, guarantee autonomy in requesting days off, and reduce the number of night work days.

Purpose: This study aimed to identify the factors affecting the suicidal ideation among older adults in the community. Methods: Data were collected from 135 older adults living in C, S, and J cities, aged 65 years or older, from March 8 to April 30, 2021, using structured questionnaires. The data were analyzed using the SPSS 23.0 program for descriptive statistics, independent t-test, one-way ANOVA, Kruskal-Wallis, Scheffé test, Pearson's correlation analysis, and hierarchical regression. Results: The mean score for suicidal ideation was 5.27±5.86. The significant predictors of suicidal ideation in older adults were activities of daily living (β=.28, p<.001), loneliness (β=.27, p=.003), self-rated health (β=-.23, p=.004), depression (β=.19, p=.047), and the existence of a spouse (β=.19, p=.009), with an explanatory power of 33.0%. Conclusion: Based on this study, we suggest that suicidal ideation in the elderly can be reduced by implementing a psychological program. Where there is no living spouse in the older adults' lives, such a program would improve physical health and reduce loneliness and depression.
