Background: Psoriasis is a T cell-mediated disorder. Appropriate treatment of psoriasis can result in a plethora of dynamic changes in the T cells and their cytokine production. Studies on the expression of Th1 and Th17 inflammatory cytokines after various treatments have reported inconsistent results.
Objective: This study aimed to investigate the changes in inflammatory cytokine expression in psoriasis skin lesions before and after psoriasis treatment.
Methods: Five patients with plaque psoriasis were enrolled in the study. The tissue mRNA and protein levels of interleukin (IL)-12, IL-2, interferon (IFN)-γ, IL-23, IL-17A, and IL-22 in psoriatic skin lesions before and after psoriasis treatment were detected using real-time polymerase chain reaction and immunohistochemical staining, respectively.
Results: The decrease in the psoriasis area and severity index was accompanied by a reduction in IL-12 and IL-23 mRNA levels and a concomitant reduction of IL-2 and IL-17A protein levels in psoriasis skin lesions. Both mRNA and protein levels of IFN-γ in psoriatic skin lesions increased after treatment.
Conclusion: The mRNA levels of tissue inflammatory cytokines (IL-12 and IL-23) and the protein levels of tissue inflammatory cytokines (IL-2 and IL-17A) significantly decreased after psoriasis treatment. The mRNA and protein levels of IFN-γ increased, even after psoriasis treatment. (Korean J Dermatol 2022;60(7):409∼413)

Background: Postoperative scars following cleft lip surgery are frequently accompanied by cosmetic and functional problems; therefore, patients with cleft lip scars usually undergo various long-term scar treatment courses. Currently, only a limited number of fractional laser treatment cases of cleft lip scars have been reported.
Objective: This study aimed to evaluate the safety and efficacy of the combination laser treatment for hypertrophic scar management following cleft lip surgery.
Methods: This single-center retrospective study included 27 patients who underwent cleft lip surgery before 12 months of age. The control group comprising 13 patients received only a silicone gel or sheet for the scars, while the treatment group comprising 14 patients underwent laser treatment with a 1,550 nm non-ablative fractional erbium-glass laser, followed by a 10,600 nm ablative fractional carbon dioxide laser. Intralesional triamcinolone injection (5∼10 mg/mL) was administered immediately after laser treatment. Scar improvement was assessed at 1, 3, and 6 months after surgery using the Vancouver Scar Scale.
Results: The mean total Vancouver scar scale showed a statistically significant improvement in the treatment group compared to that in the control group. In the sub-analysis of pigmentation, the vascularity, pliability, and height significantly improved in the treatment group. No differences in the patients' baseline characteristics between the two groups were noted. No significant adverse effects were observed during the follow-up.
Conclusion: Combination laser therapy using fractional lasers and intralesional triamcinolone injection is safe and effective for managing cleft lip surgery scars in infantile patients. (Korean J Dermatol 2022;60(7):414∼419)

Background: Biologics-experienced patients are more likely to show a lower response to biologics than that of biologic-naïve patients. However, no consensus on switching biologics exists.
Objective: We aimed to investigate the switching patterns and efficacy of the switched biologics in patients with moderate-to-severe psoriasis in actual clinical practice.
Methods: This multicenter retrospective study included 37 patients with a history of switching biologics. We analyzed the reasons for switching, the switching patterns, and psoriasis area and severity index (PASI) 75 response rates after switching biologics. We also analyzed the factors affecting the PASI75 response rate to the second biologic.
Results: The reasons for switching baseline biologics were primary failure in five patients (13.5%), secondary failure in 28 patients (75.7%), and adverse events in four patients (10.8%). The second biologics prescribed mostly include interleukin (IL)-23 inhibitor in twenty-four patients (64.9%), IL-17 inhibitor in eight patients (21.6%), tumor necrosis factor-α inhibitor in three patients (8.1%), and IL-12/23 inhibitor in two patients (5.4%). A total of 46% of patients (17/37) switched biologics from IL-12/23 inhibitors to IL-23 inhibitors. The PASI75 response rates at the primary endpoint of the second and third biologics were 89.2% and 88.8%, respectively. Our study found that female sex and obesity were associated with the primary failure of the second biologic.
Conclusion: Secondary failure was the most common reason for switching baseline biologics. Korean dermatologists prefer different classes of biologics while switching. The PASI75 response rates at the primary endpoints of the second and third biologics were relatively satisfactory. (Korean J Dermatol 2022;60(7):420∼428)

Background: Green nail syndrome (GNS) is characterized by a greenish discoloration of the nail with accompanying nail apparatus disorders, such as onycholysis or paronychia. To date, data on the clinical characteristics of GNS are limited, especially in Korea.
Objective: This study aimed to investigate the clinical characteristics of GNS.
Methods: We retrospectively reviewed the medical records and clinical photographs of 78 patients with 91 GNS lesions diagnosed at the Pusan National University Hospitals (Busan and Yangsan) from 2009 to 2021.
Results: Among 78 patients, 47 (60.3%) were female and 31 male (39.7%), with a mean age of 53.1 years (23∼82 years). This study included 56 fingernail (61.5%) and 35 toenail lesions (38.5%). Thumbnails and great toenails were the most common site, with 62 lesions (68.1%). Most patients presented predisposing conditions, such as frequent exposure to wet conditions (55.1%), immunosuppressive conditions (26.9%), and trauma history (29.5%). The most common colors, shapes, and eccentricities of GNS were blackish-green (31.9%), reverse triangle (53.8%), and distal eccentricity (42.9%), respectively. The GNS was accompanied by various nail diseases that mostly (80.2%) preceded the syndrome, and onycholysis was the most common (83.5%). Based on the involvement of the nail surface or subsurface, GNS can be divided into the superficial (15.4%), subungual (73.6%), and mixed (11.0%) subtypes, each of which shows different clinical features, such as multiplicity, color, shape, and eccentricity.
Conclusion: This is the largest-scale study showing the clinical characteristics of GNS and can be helpful for dermatologists who usually and primarily treat GNS. (Korean J Dermatol 2022;60(7):429∼435)

Setleis syndrome, also known as the focal facial dermal dysplasia type 3, is characterized by atrophic scar-like lesions in both the temporal regions and other facial abnormalities. The syndrome is associated with a mutation in the TWIST2 gene and is inherited as an autosomal recessive pattern. Here, we describe a case of an 18-year-old woman and her 10-year-old brother with bitemporal depressed patches. The lesions were observed at birth, but they were replaced by atrophic patches after several months. No abnormalities in growth or development were observed. Histopathologic findings showed atrophy of the epidermis and loss of appendages, such as hair follicles and adnexal glands, in the atrophic patches compared to that of the adjacent normal skin. Dermatofibroma (DF) is one of the most common benign soft tissue tumors. Various histological variants of DF have been described, and it is important to discern the different variants to evaluate the prognosis. A 43-year-old female patient with no specific medical history presented with multiple brown-colored nodules on the right arm for 5 years. Biopsy specimens showed a predominance of collagen and fibroblasts in a whorled arrangement with moderate cellularity, findings that were consistent with fibrocollagenous DF. The patient underwent excision of the largest lesion and revisited our department 3 months after surgery with local recurrence at the excision site. We report a rare case of multiple fibrocollagenous DF with local recurrence at the excision site. (Korean J Dermatol 2022;60(7):440∼443)

Leukocytoclastic vasculitis after the BNT162b2 vaccine and ChAdOx1 nCoV-19 Corona Virus vaccine [recombinant] has been observed. Herein, we report two cases of leukocytoclastic vasculitis that developed after the ChAdOx1-S [recombinant] vaccination. A 61-year-old and a 52-year-old woman presented with pruritic purpuric macules and papules on both lower legs. The patients had been vaccinated with the ChAdOx1-S [recombinant] vaccine. The histopathological analyses were consistent with a diagnosis of leukocytoclastic vasculitis. They were treated with oral prednisolone and improved within 1 month of treatment. We assume that the rash had arisen from the deposition of spike protein at the skin tissue induced by the viral vector of the COVID-19 vaccine or hyperimmune responses by excipients present in vaccine preparations. To our knowledge, our cases would be the first Korean cases of leukocytoclastic vasculitis after the ChAdOx1-S [recombinant] vaccination. (Korean J Dermatol 2022;60(7):444∼ 448)

Background: Desmogleins are transmembrane glycoproteins of the desmosome which provide mechanical strength to epithelial tissue. Desmogleins have so far, been implicated in several diseases such as pemphigus, striate palmoplantar keratoderma, 4S and squamous cell carcinomas. Skin cancer usually occurs in old age. And there are reports that the expression of desmogleins are increased in squamous cell carcinoma. However the role of desmogleins in skin aging has not yet been reported. Objective: The purpose of this study was to investigate the expression of desmoglein 1 and 3 according to chronologic skin aging. Methods: A total of 6 normal tissue samples from sun-protected skin of different age groups (from 34-year-old to an 84-year-old) and 1 squamous cell carcinoma tissue from a 72-year-old patient were taken. Western blotting and immunohistochemical staining were performed with anti desmoglein 1 and 3 antibodies. The expression of desmoglein 1 and 3 by Western blotting were calculated semiquantitatively by a densitometer. Results: The expression of desmoglein 1 was 0.382 in the 34-year-old, 0.450 in the 45-year-old, 0.369 in the 56-year-old, 0.761 in the 65-year-old, 1.035 in the 77-year-old and 1.329 ODu/mm2 in the 84-year-old. The expression of desmoglein 3 was 0.830 in the 34-year-old, 0.984 in the 45-year-old, 1.029 in the 56-year-old, 1.534 in the 65-year-old, 1.714 in the 77-year-old and 1.878 ODu/mm2 in the 84-year-old. In immunohistochemical staining, the expression of Dsgl increased from the basal layer to the granular layer and Dsg3 was expressed in the basal and suprabasal layers. Conclusion: The expression of desmoglein 1 and 3 were increased according to chronologic skin aging. (Korean J Dermatol 2007;45(7):645~649)

Background: Pemphigus vulgaris is an autoimmune bullous dermatoses of skin and mucosa characterized by loss of adhesion between keratinocytes, a process known as acantholysis. Apoptosis, programmed cell death, may participate in pathogenesis of intercellular detachment and loss of cell-matrix interaction. Objective: This study was designed to investigate the induction of apoptosis in the pemphigus lesional epidermis and to elucidate the mechanism of apoptosis induced by pemphigus sera. Methods: Hoechst 33342 staining was performed to determine the induction of apoptosis in the pemphigus lesional epidermis. In addition, we used HaCaT cells treated with pemphigus sera and analyzed the expression of caspase-3, caspase-8, caspase-9 and bcl-2, bcl-xL, bax, bak by the RT-PCR method. Results: In Hoechst 33342 staining, typical findings of apoptosis were observed in the pemphigus lesional epidermis showing acantholysis. RT-PCR showed the upregulation of caspase group (caspase-3, caspase-8, caspase-9), the downregulation of antiapoptotic bcl-2 family (bcl-2, bcl-xL) and the upregulation of proapoptotic bcl-2 family (bax, bak). Conclusion: These results suggest that apoptosis may be associated with acantholysis of pemphigus lesional epidermis and may play an important role in the pathogenesis of pemphigus. Background: Pigmented basal cell carcinomas (PBCC) may be confused with melanoma and other benign pigmented skin lesions. Dermoscopy is a noninvasive technique that is known to increase the diagnostic accuracy of benign versus malignant pigmented skin lesions. Menzies et al recently proposed a simple dermoscopic model for the diagnosis of PBCCs. Objective: To demonstrate whether this criteria was adequate for Korean PBCC patients and seek other characteristic dermoscopic findings in Korean PBCCs. Methods: Dermoscopic images of 50 biopsy-proven PBCCs were examined. Results: Negative features such as a pigment network were not observed in all PBCCs. Positive features - ulceration, large blue-gray ovoid nests, multiple blue-gray globules, maple leaf-like areas, spoke wheel areas and arborizing (treelike) telangiectasia were present in 39 (78.0%), 41 (82.0%), 33 (66.0%), 21 (42.0%), 1 (2.0%) and 32 (64.0%) of 50 PBCCs respectively. All PBCCs satisfied the dermoscopic criteria for diagnosis of PBCC suggested by Menzies et al. Other dermoscopic features were as follows; multiple blue-gray dots (28.0%), milia-like cysts (8.0%), granules (6.0%), diffuse blue-white area (6.0%) and hairpin vessels (2%). Forty-four % of lesions had less than 50% pigmentation of the area and 38.0% of lesions had more than 75% pigmentation. Conclusion: The criteria of PBCC proposed by Menzies et al was useful for dermoscopic diagnosis of Korean PBCCs. Ulceration and pigmentary features such as large blue-gray ovoid nests, multiple blue-gray globules, maple leaf-like areas, multiple blue-gray dots, granules and larger pigmented areas were more frequently observed in Korean PBCCs. However, other specific dermoscopic findings for diagnosis of PBCC in Koreans were not present. (Korean J Dermatol 2007;45(7):659∼665)

Background: Although basal cell carcinoma (BCC) is the most common human skin tumor, with calcification reportedly taking place in about 20% of all BCC cases, the pathogenesis of calcification in BCC has not yet been studied. Objective: The purpose of this study was to evaluate factors related to the pathogenesis of calcification in BCC. Methods: We performed immunohistochemical staining for β-catenin, frizzled-2, BMP-4, osteopontin, osteocalcin, and osteonectin using frozen skin tissue from 15 cases of BCC with calcification and 11 cases of BCC without calcification. Results: The expression of β-catenin showed positive in 14 of the 15 cases in BCC with calcification, but negative in all 11 cases of BCC without calcification. The expression of frizzled-2 was observed in 14 of the 15 cases in BCC with calcification, and in 10 of the 11 cases in BCC without calcification. The difference did not reach statistical significance (p=0.236). The expression of BMP-4 was observed in all 26 samples of BCC, but the intensity of expression did not reach statistical significance between the two groups (p=0.293). Furthermore, osteopontin and osteocalcin showed no statistical significance between two the groups (p=0.567, p=0.401). The expression of osteonectin was observed in all of the BCC cases, and was stronger in BCC with calcification than in BCC without calcification (p=0.042). Conclusion: We suggest that the calcification in BCC might be related to the increase of β-catenin expression and that osteonectin might also influence the process of calcification in BCC. Background: The stratum corneum of the skin of patients with atopic dermatitis (AD) is highly susceptible to colonization by various bacteria, including Staphylococcus (S.) aureus. Because the frequency of bacterial colonization is significantly higher, skin barrier function against bacterial invasion appears to be significantly disrupted in AD. S. aureus colonization in AD is thought to be important in the development and aggravation of eczematous skin lesions. Over the past decades, there have been several quantitative methods of cutaneous bacterial culture from the skin; the swab method, scrub method, tape method and contact plate method. Objective: The purpose of this study was to evaluate the effectiveness of the contact-plate sampling technique as a quantitative culture method of S. aureus in atopic dermatitis. Method: The subjects were 24 patients with AD and 24 age-matched normal controls. S. aureus was sampled at 4 evaluation sites (neck forearm abdomen, popliteal fossa) of AD patients and normal controls using the contact-plate sampling technique. We also checked skin barrier function by measuring the transepidermal water loss (TEWL). Results: Based on the data, we found that the number of S. aureus colonies in the samples from AD patients was significantly higher than from normal controls (p<0.05, 4 sites respectively). Also, TEWL at the 4 evaluation sites of AD patients was significantly higher than in normal controls (p<0.05, 4 sites respectively). However, S. aureus colonization did not usually show significant correlation with TEWL. Conclusion: The contact-plate sampling technique provides a reliable qualitative and quantitative culture method of S. aureus, which is inexpensive and convenient so can be used in both children and within a busy clinic. Background: Chronic idiopathic urticaria (CIU) has a major impact on patients` quality of life. However the management of CIU has sometimes been challenging to physicians, with little response to conventional therapy like antihistamines. Objective: This study was aimed to determine the effectiveness and safety of low-dose cyclosporine for the treatment of recalcitrant CIU with a positive autologous serum skin test (ASST). Methods: Thirty CIU patients who were unresponsive to conventional antihistamine therapy were treated for 3 consecutive months with low-dose cyclosporine (2∼3 mg/kg/day) and fexofenadine (180 mg/day). The clinical efficacy was measured at baseline, week 1, 2, and month 1, 2 and 3. CIU sign and symptom scores were assessed using 4-point scales for pruritus, number of hives, size of largest hive, interference with sleep, and interference with daily activities. Global assessment of the severity by patients and any side effects were also measured at every visit. Results: Low-dose cyclosporine significantly improved the total CIU scores. In addition, the interference with sleep was reduced and the performance of daily activities improved in accordance with patient`s global assessment of urticaria severity after 3 month of treatment. Moreover there were no significant side effects to suggest stopping cyclosporine administration. Conclusion: These results show that low-dose cyclosporine therapy might be a good therapeutic alternative in CIU patients who are unresponsive to conventional treatments. CD 30 positive anaplastic large cell lymphoma (ALCL) is a type of non-Hodgkin`s lymphoma, a tumor that is composed of mostly mast cells expressing CD30 antigens. According to the 2005 World Health Organization-European Organization for Research and Treatment of Cancer (WHO-EORTC) classification, it can be classified as systemic ALCL and primary cutaneous CD30 positive T-cell lymphoproliferative disorder. Systemic ALCL can be subclassified into ALK positive and negative. There have been many cases of primary cutaneous CD30 positive T-cell lymphoproliferative disorder, but reports of systemic ALCL have been rare. We report a case of CD30 positive/ALK negative systemic anaplastic large cell lymphoma involving bone marrow in a 51 year-old-male who showed two pruritic, 1.5×1.0×0.8 cm and 1.0×0.5×0.4 cm sized, erosive, erythematous nodules on the right thigh and calf. Recently another distinct subtype of lymphoma has been discovered, the natural killer (NK)T cell lymphoma, which can involve the skin in a primary or secondary fashion. NK/T cell lymphoma is characterized by the biphenotype expression of the T-cell marker and NK cell marker, especially CD56. These CD56+ lymphomas are further subdivided into nasal NK/T-cell lymphomas and non-nasal NK/T-cell lymphomas. We report a case of primary nasal NK/T cell lymphoma, which showed as an atypical cellulites-like lesion on the face. The patient had suffered with recurrent orbital swelling for several years before diagnosis. (Korean J Dermatol 2007;45(7):692∼696)

Palisaded neutrophilic granulomatous dermatitis (PNGD) is an unusual entity that has been described under several different names including, linear subcutaneous bands, interstitial dermatitis with cutaneous cords and arthritis, rheumatoid papules and Churg-Strauss granuloma. The condition is seen in patients with rheumatoid arthritis and other connective tissue diseases. PNGD shows variable clinical manifestations and represents a histologic spectrum, starting with early vasculitic lesions, then palisaded granulomatous lesions and dermal fibrosis with minimal leukocytoclastic debris in the late stage lesions. We report an unusual case of a 49-year old woman with rheumatoid arthritis who presented with multiple acral nodules. The histologic examination revealed palisaded neutrophilic granulomatous dermatitis. (Korean J Dermatol 2007;45(7):697∼701)

Muir-Torre syndrome (MTS) is a rare autosomal dominant genodermatoses, first described in 1967. It is characterized by the presence of sebaceous tumors and an internal malignancy in the absence of other predisposing factors. The most common internal malignancy associated with MTS is colorectal adenocarcinoma, although a wide tumor spectrum exists including carcinomas of the genitourinary tract, breast, and hematologic malignancy. It is rare for more than two internal malignancies to occur in MTS. We report a rare case of MTS with B-cell lymphoma, esophageal cancer and gastric malignancy in a 63-year-old male.
