Purpose: This study aimed to explore the experiences of nursing students' therapeutic use of self in web-based psychiatric nursing practicum focusing on role-playing. Methods: The participants were 13 senior nursing students who had underwent three weeks of web-based psychiatric nursing practicum. Data were collected through in-depth video interviews, and analyzed using the grounded theory method, developed by Strauss and Corbin (1998). Results: In total, 14 categories and 29 sub-categories were generated. The central phenomenon was “hardship in the therapeutic use of self.” The action/interaction strategies were “implementation of role-playing considering therapeutic communication,” “heightened sensitivity about nursing priorities,” and “performing relearning and repetitive training”. The consequences of these action/interaction strategies went through “further development of the therapeutic use of self,” “recognizing that persistent self-reflection is required”, and that the participants “gained confidence about the therapeutic use of self.” The core category of the experience of therapeutic use of self was to “overcome hardship and confidently advance forward.” This process manifested in the stages of burden, crouching, urgency, and confidence. Conclusion: The findings suggest that more attention needs to be paid to the therapeutic relationship and the therapeutic use of self in education and research.

Purpose: The purpose of this study was to examine the level of Person-centered Critical Care Nursing (PCCN) and the factors influencing PCCN for nurses in Intensive Care Units (ICU). Methods: This study was designed by cross-sectional descriptive correlational study. The participants included 147 ICU nurses in two general hospitals in Seoul, Korea. Demographic characteristics, PCCN, communication skills, professionalism, and work environment were measured. The collected data were analyzed using descriptive statistical analysis, independent t-test, One-way ANOVA, Pearson’s correlation coefficient, and stepwise multiple linear regression with the SPSS/Win 25.0 program. Results: The average age of the participants was 29.6±4.7 years and the mean work experience in the ICU was 4.67±3.52 years. The level of PCCN was 3.70±0.41, which was moderate to high, and it significantly showed a positive correlation with therapeutic communication skills (r=.66, p<.001), global interpersonal communication competence (r=.42, p<.001), professionalism (r=.38, p<.001), and work environment (r=.16, p=.048). The factors influencing PCCN were identified as therapeutic communication skill and global interpersonal communication competence (Adj R2=.45, p<.001). Conclusion: The findings of this study were confirmed that the strategies to promote PCCN are necessary to enhance therapeutic communication skill and global interpersonal communication competence. In addition, they may be particularly meaningful in providing basic data for nursing education and future intervention development research to promote PCCN for the ICU nurses. For improving PCCN for healthcare providers in ICU, further studies should be conducted to develop education and intervention programs.

Purpose: This study aimed to describe clinical nurses’ lived experiences of workplace verbal violence through qualitative research using descriptive phenomenology. Methods: Six female Korean nurses who had less than 5 years of clinical experience and had experienced verbal violence in the workplace within the past year participated in the study. Data were collected through one-on-one in-depth interviews with the participants and analyzed using Colaizzi’s phenomenological method. Results: A total of 27 codes, eight themes, and four theme clusters were derived from the participants’ statements. The four theme clusters of the clinical nurses’ experiences of verbal violence in the workplace were as follows: “tip of the iceberg,” “beyond me and my control,” “fear and resignation,” and “personal burden.” The participants recognized that nurses experienced verbal violence daily, and that the causes of and responses to verbal violence were determined by external situational factors rather than nurses’ individual problems. This suggests that nurses felt that they had no choice but to personally cope with verbal violence and bear the consequences due to systematic indifference and silence about verbal violence experienced by clinical nurses. Conclusion: The findings show that verbal violence was pervasive and unmerited, yet often endured at the cost of a personal burden to nurses. A clear definition of verbal violence and education for employees are needed, and a reporting system should be established to report all forms of violence regardless of the severity of the incident.

Purpose: This study was conducted to explore the relationships among new nurses’ meaning of work, organizational commitment, professional self-image, and retention intention through path analysis. Methods: The participants were 872 nurses with less than a year of clinical experience working at general or tertiary hospitals over 500 beds in Korea. The data were collected using structured questionnaires from May 15, 2021 to June 28, 2021, and analysis was conducted using SPSS/WIN 22.0 and AMOS 21.0 programs. Results: Retention intention was explained by meaning of work, organizational commitment, and professional self-image, its explanatory power was 46.5%. The mediating effect of professional self-image was found on the relationship between meaning of work and retention intention. Conclusion: To encourage the retention intention among new nurses, an environment where new nurses can set a high value on the meaning of work and sense of accomplishment should be created, and a strategy to improve organizational commitment and professional self-image should be established.

이 논문은 지금의 코로나 시국에 경험하는 가장 중요한 문제 가운데 하나인 ‘의료인력 부족’과 관련하여, 현재 국회에 계류 중인 간호법(안)에 대한 검토를 현행 의료법에 대한 비판적 시각에서 진행한 글이다. 글의 전반부에서는 입법의 연혁과 현황을 살펴보았다. 대략 45년 이상의 기간 동안 추진해 온 바 있는 간호법 독립의 실패 연혁을 간략히 살펴보고, 현재에도 변함없는 찬반론의 주요 쟁점에 대하여 검토하였다. 간호 업무의 종속성 문제, 전문간호사 제도의 실효성 문제, 간호조무사의 지위 문제 등이 그것이다. 글의 중반부에는 이렇듯 대립하는 쟁점들과 관련하여 현행 의료법과 계류 중인 간호법(안)의 내용들에 대하여 구체적인 검토를 진행하였다. 특히 현행 의료법이 가지고 있는 문제점을 간호법(안)을 통하여 얼마나 해결할 수 있는지 상세히 분석하는 것이 주요한 연구의 내용이었으나, 다소 실망스러운 결과를 얻었다. 간호사의 업무 범위와 처우 개선의 문제에 대하여는 간호법(안) 제정으로 실효성을 기대해 볼 수 있겠으나, 전문간호사나 간호조무사에 대한 정책은 ‘해결이 아닌 외면’으로 규정되어 있기 때문이다. 글의 후반부에는 이렇듯 의료 현실을 외면한 지나친 신중론에 대한 비판적 검토를 진행하였다. 의료체계에 존재하는 수직적인 구도와 여성이 절대 다수인 간호사에 대한 권력적 편견, 그리고 이권의 고수와 경제성의 논리로 일관하고 있는 현행 의료시스템상의 문제점들을 주요 쟁점을 기준으로 정리하여 비판적으로 분석하였다.


This article is a review of the Nursing Act (bill) currently pending in the National Assembly, in relation to the “shortage of medical personnel” which is one of the most important problems experienced in the current Corona situation, based on a critical perspective on the current medical law. The first part of this article is allotted to reviewing the history and current status of legislation. The history of the failure of nursing law independence, which has been promoted for more than 45 years, is briefly reviewed, and the main issues of pros and cons that remain unchanged even today are reviewed. These are the dependency problem of nursing work, the effectiveness problem of the professional nurse system, and the status problem of the nursing assistant. In the middle of the article, in relation to these conflicting issues, the contents of the current Medical Service Act and the pending Nursing Act (bill) were reviewed in detail. In particular, the main study was to analyze in detail how much the problems of the current Medical Service Act can be solved through the Nursing Act (bill), but the results were somewhat disappointing. The effectiveness of the enactment of the Nursing Act (bill) can be expected with respect to the problem of the scope of work and improving treatment of nurses, but the policy for professional nurses and nursing assistants is prescribed as ‘not to solve, but to ignore’. In the last part of the article, a critical review of the excessive prudence that ignores the medical reality was conducted. The vertical structure that exists in the medical system, the power bias toward nurses, who have an absolute majority of women, and the problems in the current medical system, which are consistent with the logic of adherence to interests and economic feasibility, are summarized and critically analyzed based on major issues.

6간호조무사 직무분석을 통한 대한민국 간호조무사 자격시험 항목

배경 : 우리나라 간호조무사 직무를 분석하여 간호조무사 자격시험 개선 사항을 의논하고자 한다. 방법 : 질적 연구 방법으로 간호조무사 직무, 특성을 이해하기 위하여 관련한 논문과 보고서를 조사하였고, 간호조무사 관련 전문가들(특성화고등학교 교사, 간호학 교수, 간호조무사 양성학원, 의사, 한의사)과 세미나 및 델파이 조사를 통하여 심도있는 간호조무사의 직무 특성 및 자격증 취득에 관하여 확인하였다. 추가로 간호조무사 자격시험 개선을 위한 포괄적 의견 수렴을 위하여 포커스 그룹 인터뷰를 10명의 간호사와 7명의 간호조무사 양성 교사 및 학원장 참여로 시행하였고, 또한 양적 연구 방법으로 256명의 간호사 및 간호조무사 대상의 온라인 설문 조사를 통해 질적 연구 결과를 확인하는 체계로 하였다. 결과 : 연구 결과 우리나라 간호조무사의 직무 범위는 상당히 포괄적이고 다양하였으나, 현행 간호조무사 자격시험이 임상에 요구되는 전문적 직무 수행을 위하여서는 한계가 있었다. 연구 결과 간호조무사 자격시험은 직무기반 개선의 필요성이 있고 이론적 학문 기반 평가보다는 “감염 예방”, “응급 대응” 등 실무 중심에 기반하여 문제 출제가 이루어질 필요가 있다는 의견이 다수였다. 결론 : 간호조무사 자격시험 개선을 위해서는 간호조무사가 실질적으로 행하여지는 분야(감염과 응급)에서의 시험 출제 강화 및 교육이 이루어져야 임상에서(1차 병원 및 환자) 높은 요구도를 만족시킬 수 있겠다.


Background : It aimed to analyze the duties of nurse assistants and to provide recommendations for improving the Korea Nurse Assistant Licensing Examination (KNALE). Methods : In order to understand the job characteristics, job content and requirements and KNALE topics were reviewed. Furthermore, the experts’ opinions concerning nurse assistants (specialized high school teachers, nursing and health professors, nursing academy directors, physicians, and oriental medicine doctors) were assessed. A seminar for convergence, a focused group interview with 10 nurses and 7 health scientists, was done. The online questionnaire for 265 nurses, nurse assistants, health science majors, and medicine majors were also conducted. Results : The survey from experts and nurse assistants revealed that the job characteristics of a nurse assistant are diverse and that the KNALE is structured in a way that does not match the professionalism required on the field. The job-based exam items should be included to improve the KNALE. Specifically, the number of practice-based items should be greater than that of theory-based items and should emphasize on “infection and injury” and “emergency response” areas. The inclusion of a former nurse assistant who can contribute to items production and item review is highly favored to improve the items’ suitability to nurse assistant’s job. Conclusion : It is necessary to improve the examination items by placing a greater emphasis on areas of “infection and injury” and “emergency response,” because a high level of such knowledge is demanded from nurse assistants who work in hospitals.

본 연구의 목적은 국민건강보험 간호간병통합서비스의 적용을 받는 병동에서 간호인력 배치 수준이 임상현실에 적용됨에 있어 나타나는 문제점을 확인하고, 대안적 배치기준을 개발하는 것이다. 국민건강보험공단의 간호간병통합서비스 관련 통계 자료에 근거하여 간호사와 간호조무사의 실제 배치수준과 지정배치기준을 Ratio 분석법으로 평가하였다. 연구결과 196개 기관 중 약 40%에 해당하는 의료기관이 간호사와 간호조무사를 지정기준의 임계값에 근사하게 고용하는 것으로 나타났다. 이는 병상가동률의 변화, 휴직과 이직으로 인한 간호인력 고용 변화에 유연하게 대처하기 어렵다는 한계를 내포한다. 이러한 문제를 보완하기 위하여 간호사와 간호조무사 각각에 대해 설정된 현행 인력배치기준을 간호인력 1인당 환자수(ratio)의 개념으로 일원화하고, 간호사 비율에 따라 수가를 세분화하는 대안적 배치기준을 개발하였다. 이는 변동성이 높은 입원수요와 간호인력 수급 상황에 효과적으로 대응할 수 있고, 건강보험공단의 관리 효율성을 제고할 수 있는 장점이 있다. 의료기관 경영 측면에서는 이윤극대화를 위하여 간호사를 간호조무사로 대체하려는 부작용이 나타날 수 있으므로 엄격한 모니터링과 강화된 벌금제도가 필요하다.


This study is aimed to evaluate the adequacy of staffing level in the integrated nursing care service(INCS) in South Korea and to develop a new INCS staffing level. The National Health Insurance Service(NHIS) data on INCS staffing statistics and yearly claimed data were used to identify actual nursing personnel staffing. The ratio of actual staffing level to designated staffing level was calculated using the gathered data. As a result, a total of 196 hospitals were collected for analyzing the ratio of actual staffing to designated staffing. Approximately 40% of the hospitals’ employed nurses and nursing assistants were close to the threshold of the designated staffing level. This implies that it is difficult to cope with the fluctuations of the number of patient and unexpected leave and resignation of the nursing personnel. To make up for those weak-points, a new staffing standard that unify the nurse and nurse assistant staffing level into the nursing personnel staffing level and subdivide the staffing level and medical fee according to the registered nurse retention ratio was developed. The new staffing standard can flexibly cope with fluctuation in severity of ward patients and ward conditions and also improve management efficiency of NHIS. However to prevent its side effect that replacing nurses with nurse assistants, it is required to execute a stringent monitoring and penalty system.

8임상간호사의 교대제 개선을 위한 예측 가능한 패턴형 근무제

우리나라 간호사의 낮은 인력 배치 수준과 열악한 교대근무 환경은 업무부담 및 직무스트레스를 높여 이직의도를 높일 뿐만 아니라 간호서비스의 질을 저하시켜 환자안전을 위협하고 있다. 본 연구는 임상간호사의 교대제 관련 선행연구, 근무일정표 분석, 간호사-전문가 그룹토론 및 패널 조사를 통하여 현장에서 적용 가능한 교대제 개선모델을 개발하고 모델의 타당도를 평가하였다. 본 연구 결과, 교대제 개선을 위해 병원종별, 환자의 중증도에 따른 상이한 ‘예측 가능한 패턴형 근무제’를 제시하였다. 모델의 타당도를 평가하기 위한 델파이조사는 2021년 8월 25일부터 10월 8일까지 간호사, 전문가 16명을 대상으로 시행하였고 모델에 대한 내용타당도를 조사한 결과 ‘동일야간 근무일수’를 제외하고 0.88~1.00의 범위로 모두 타당한 것으로 나타났다. 따라서 임상 간호사 교대제 개선을 위한 모델이 예측 가능성을 갖기 위해서는 적정 인력을 전제로 적정 근무, 적정 휴가가 배치되어야 함을 확인하였다.


A low staffing ratio and poor shift work environment not only increase the turnover intention by increasing job stress in Korean nurses but also threaten patient safety by lowering the quality of nursing services. In this study, a predictable pattern shift schedule model was developed by reviewing previous researches, an analysis of nurses` shift schedules, and group discussions with nurses as well as experts in the field of nursing and related labor issues. Validity and feasibility of the model was evaluated using the Delphi survey technique with 16 panels from August 25 to October 8, 2021. Results indicated that the developed pattern shift schedule can be applicable to different hospital types and patients’ severity of symptoms. The content validity index (CVI) of the model ranged from 0.88-1.00 except for the item “equal number of night shift work” (CVI=0.69). Thus, appropriate staffing ratio level is a prerequisite for decent work and decent leaves.

Purpose: The purpose of this study was to investigate the influence of nursing competencies and military spirit on disaster preparedness among Korea Armed Forces Nursing Academy (KAFNA) cadets. Methods: This descriptive study was conducted from the 3rd and 4th grade KAFNA cadets. The data were collected from August 17 to 20, 2021. The structured questionnaire consisted of 106 questions about nursing competencies, military spirit, disaster preparedness, and general characteristics of participants. Results: The average score of nursing competencies, military spirit, and disaster preparedness were 3.92±0.48, 3.71±0.61 and 3.35±0.60 out of 5. Disaster preparedness was positively correlated with nursing competencies (r=.62, p<.001) and military spirit (r=.38, p<.001). There was significantly positive correlation between nursing competencies and military spirit. (r=.51, p<.001). The significant influencing factors on disaster preparedness were ‘nursing competencies’ (β=0.51, p<.001) and ‘4th grade’ (β=0.59, p<.001). Conclusion: To enhance the disaster preparedness of cadets in response to disasters, an education program for KAFNA cadets should be focused to improve the nursing competencies based on disaster nursing education.

10육아휴직 후 복직 간호사의 양육스트레스, 일-가정 갈등, 자아탄력성이 재직의도에 미치는 영향

Purpose: This study investigated whether parenting stress, work-family conflict, resilience affect retention intent in Korean nurses returning to work after parental leave. Methods: The participants in this study were 111 nurses recruited from 10 hospitals in Korea, who were working after formal parental leave from their hospitals. Collected data were analyzed using descriptive statistics, the t-test, analysis of variance, the Mann-Whitney U-test, Pearson correlation coefficients, and hierarchical regression. Results: Retention intention (33.80±7.78), parenting stress (101.70±17.57), and resilience (85.02±12.75) were at greater than moderate levels and a midpoint level of work-family conflict (29.63±7.00) was noted in this sample of mostly women nurses in their 30s. The factors affecting retention intent were parental leave duration, the number of times that participants had taken parental leave, health condition, work-family conflict, and resilience. The total explanatory power of these variables was 36.7%. Retention intent had a negative correlation with parenting stress and work-family conflict. Conversely, retention intent was positively correlated with resilience. Conclusion: This study supports the need for flexible adjustment of returning nurses’ working hours and family-friendly policies to promote balance between work and family. It is also necessary to develop and apply measures that boost resilience and support health improvement for nurses returning to work.As nurses are often assigned to new areas of work upon return, training programs to aid their adjustment may also be helpful.
