본 연구는 신경발달장애 영유아를 대상으로 한국판 아동기 자폐평정척도 2판 표준형(K-CARS2-ST)의 요인구조를 탐색하고, 진단 집단, 성별, 연령에 따른 차이를 분석하여 요인구조의 타당성 및 임상적 유용성을 검증하였다. 서울 및 경기도 소재의 종합병원, 정신건강의학과, 아동발달센터에서 전향적·후향적으로 수집한 자폐스펙트럼장애(ASD) 211명, 의사소통장애(CD) 143명, 지적발달 장애(IDD) 137명을 포함한 만 2~6세 영유아 491명의 K-CARS2-ST 자료를 활용하여 주성분분석, 공 분산분석, 독립표본 t 검정을 실시하였다. 요인 분석 결과, “사회적 의사소통 및 상호작용의 결함”과 “제한적·반복적 행동 및 감각 이상”의 2요인구조가 도출되었으며, 신뢰도는 .82와 .76으로 양호하였다. 연령과 성별을 공변량으로 통제한 공분산분석 결과, 두 요인 점수와 총점 모두에서 ASD 집단이 IDD, CD 집단보다 유의하게 높은 점수를 보였다. 독립표본 t 검정 결과, “제한적·반복적 행동 및 감각 이상” 요인에서만 남아가 여아보다 유의하게 높았고, 36개월 이상 집단이 36개월 미만 집단보다 유의하게 높았다. 본 연구는 K-CARS2-ST와 DSM-5의 ASD 진단기준의 일치도를 확인하여 도구의 임상적 유용성을 검증한 기초연구이며, 조기 감별 및 중재 전략 수립을 위한 실증적 근거를 제공하였다.

This study examined the factor structure and clinical utility of the Korean Childhood Autism Rating Scale, Second Edition-Standard Version (K-CARS2-ST) in young children with neurodevelopmental disorders. Data from 491 children aged 2-6 years, including 211 with autism spectrum disorders (ASD), 143 with communication disorders (CD), and 137 with intellectual developmental disorders (IDD) were analyzed using principal component analysis, covariance analysis, and independent t-tests. The results thus obtained supported a two-factor structure: “Social Communication and Interaction Impairments” and “Restricted, Repetitive Behaviors and Sensory Abnormalities” with internal reliabilities of .82 and .76. The ASD group scored significantly higher than other groups on both total and factor scores. Boys scored higher than girls, and children aged 36 months or older scored higher than younger children only on the “Restricted, Repetitive Behaviors and Sensory Abnormalities” factor. These findings confirm the consistency of the K-CARS2-ST with DSM-5 criteria, supporting its use for early differential diagnosis and intervention planning.
