연명의료결정제도가 2018년도부터 시행되고 어느덧 4년이라는 시간이 흘렀다. 연명의료계획서 작성자 수도 꾸준히 증가하여 2022년 기준 8만 명 이상이 연명의료 계획서를 작성하고 있다. 연명의료중단등결정 및 그 이행에 관한 업무를 수행하려는 의료기관에 설치된 의료기관윤리위원회는 심의 및 해당 교육, 상담 등의 업무를 수행함에도 불구하고 여전히 적절한 질관리가 이루어지고 있지 않다. 특히 현장의 의견을 반영한 세부적인 평가지표의 부재로 인하여 이를 관리하는 감독기관에서도 많은 어려움이 있다. 이에 본 연구에서는 관련 전문가들을 패널로 섭외하여 의료기관윤리위원회의 질을 평가하는 데 있어서 필요한 지표를 델파이조사기법을 활용하여 도출해내고자 한다. 연명의료 관련하여 환자의 최선의 이익을 보장하고 자기결정을 존중하여 인간으로서의 존엄과 가치를 보호하기 위해, 최종적으로 의료기관윤리위원회의 질 관련 지표로는 의료기관윤리위원회의 설치, 구성, 심의 활동, 상담 활동, 교육 활동, 통계 분석의 6가지 영역이 선정되었다. 선정된 지표를 실제 의료현장에 적용하여 의료기관윤리위원회의 질을 체계적으로 평가한다면, 국민들이 삶을 존엄하게 마무리하는 데 도움이 될 것이다.

The life-sustaining treatment decision system has been in effect since 2018, and four years have already passed. As of 2022, the number of life-sustaining treatment plans has steadily increased, and more than 80,000 people are preparing life-sustaining treatment plans. The Medical Institution Ethics Committee established in medical institutions seeking to perform tasks related to the decision and implementation of life-sustaining treatment is still not properly quality assurance despite deliberation, education, and counseling. In particular, there are many difficulties in the supervisory authority that manages the lack of detailed evaluation indicators reflecting the opinions of the field. Therefore, this study aims to derive the indicators necessary to evaluate the quality of the medical institution ethics committee by recruiting related experts as panels using the Delphi survey method. In order to ensure the best interests of patients and respect self-determination in relation to life-sustaining treatment, six areas of ethics committee establishment, composition, deliberation activities, counseling activities, and statistical analysis were finally selected. Applying the selected indicators to the actual medical field to systematically evaluate the quality of the medical institution ethics committee, it will help the people finish their lives with dignity.
