Tubo-ovarian abscess (TOA) is a common serious complication of pelvic inflammatory disease (PID), usually caused by an ascending sexually transmitted infection. PID and TOA are rarely seen in virginal adolescent females. We herein want to document rare cases of pelvic inflammatory disease (PID) and tubo-ovarian abscess (TOA) in non-sexually active female adolescents. The first case of Herlyn-Werner-Wunderlich (HWW) syndrome complicated TOA is described. The second case with a history of appendectomy for perforated appendicitis presented with recurrent pelvic abscesses. The women were not sexually active. These cases remind us that PID and TOA should be kept in mind even in non-sexually active girls, especially in the presence of obstructive mullerian anomaly and a history of perforated appendicitis.
