Delayed interval delivery means delay the day of the premature delivery up to two or more days to increase the birth weight and the gestational age in twin pregnancy. We describe a case of intentional delayed interval delivery of dichorionic diamniotic twins following premature rupture of membrane at 25+4 weeks of gestational age. Spontaneous labor was occurred at 26+3 weeks of gestational age and the first baby was born vaginally after completion of betamethasone treatment for lung maturation. After the first baby was delivered, we used high dose of combined tocolytics and performed emergent cervical cerclage to delay the second baby delivery. We can prolong 7 weeks of gestational age until 33+2 weeks. First baby has been treated for RDS and sepsis, second baby has no significant complications and discharged after 23 hospital days. We suggest that delayed interval delivery is considerable choice for achieving better prognosis for the second baby when the first baby is delivered in the extremely premature period even though still on the viable weeks.
