Endometriosis is a common gynecologic disease, and affects about 5-10% of reproductive aged women. Among the different types of endometriosis lesions, ovarian endometrioma present in 20-40% of women with endometriosis and often associated with a more severe form of endometriosis resulting anatomic distorsion and infertility. For the management of ovarian endometrioma, conservative laparoscopic surgery to eliminate endometriotic lesions and restore a normal anatomy is the treatment of choice in reproductive aged women. In addition, as recurrence rate of endometrioma after surgery is high, postoperative medical treatments including GnRH agonist, oral contraceptives, and progestins etc are critical to prevent or delay the endometrioma recurrence, especially, in young reproductive aged women who plan for a pregnancy in the future. In this presentation, clinical experience of postoperative managements regarding regimen, efficacy, adverse effects, quality of life, and duration of treatment will be discussed.
