Objective Multidisciplinary team care approach is associated with improved glycemic control and better clinical outcomes of care for patients with diabetes. We investigated whether a structured intensive diabetic education program has more benefits for the prevention of hypoglycemic episodes in patients with type 2 diabetes mellitus.
Methods A total of 40 patients with type 2 diabetes (age 54.7 ± 10.2 yrs, diabetic duration 4.2 ± 7.0 yrs, A1C 10.4 ± 2.6%, 43% male) were randomized. Structured group education program was implemented for all participants. After then, intensive individual education program aiming at prevention and appropriate management of hypoglycemia was applied for intensive education group (IE).
Results After a 3-month follow-up, A1C levels were significantly decreased in both groups after education (control vs. IE group at 3 months, 7.6 ± 0.6 vs. 7.2 ± 1.7%, P = 0.571). The number of hypoglycemic episode were significantly lower in IE group compared to those in the control group (2.95 ± 3.7 vs. 1.14 ± 1.4 events, P = 0.04). Most common cause of hypoglycemia was skipping meals. In IE group, detection of low blood glucose, number of blood glucose monitoring, treatment of hypoglycemia, and dose adjustment of their hypoglycemic medications improved significantly.
Conclusion Compared to control group, IE group demonstrated additional benefits regarding to reducing hypoglycemic episodes, detecting low blood glucose, and improving self-care ability for hypoglycemia.
