Objective Use “The continuous model of innovative health education” to observe patients’ satisfaction, diet control and the impact of glycemic control. can effectively promote nutrition counseling participation rate, and thus achieve glycemic goals. Methods The subjects from the medical center of the southern Taiwan should join in the project for one year. They joining in the project under three years and receiving at least two times of health education every year in the average. There are three stages of the health education in its design. Its design focuses on the lesson plan with multi-color cards for the various dietary and healthy education of each stage. Use “The continuous model of innovative health education” to observe patients’ satisfaction, diet control and the impact of glycemic control. Results Nutrition counseling curriculum repeatability, after intervention by the new types of courses to improve satisfaction difference was 66.6%. Changes in nutrition counseling curriculum to improve sexual satisfaction difference was 53.4% and expected level of 90%. Satisfaction with overall nutritional health education before and after the upgrade from 73.4 percent to 88.7 percent, 92% target achievement, progress rate of 20.8%. Conclusion Nutrition counseling health education curriculum by new types of intervention can improve type 2 diabetes patients with long-term participation in nutrition counseling, health education curriculum complain too little variability. Take the results to diverse materials and processes used in various stages of nutrition counseling, build interactive mechanism with patients and their families, can effectively promote nutrition counseling participation rate, and thus achieve glycemic goals.
