Self-management control of the patients with type 2 diabetes mellitus in Mongolia

Objective: To evaluate the diabetes self-control and Effectiveness of the diabetes education for patients with T2DM. Methods: The cross-sectional and cohort survey was conducted Ulaanbaatar. For the study we randomized 188 patients, 150 newly diagnosed with T2DM. Our study was conducted with the approval the Medical ethics committee of the Ministry of Health, Mongolia. Each participant gave written informed consent and the study protocol was approved. We studied patient's medical examination chart. In the cohort study, we evaluated patient’s diabetes knowledge, IPQ, depression, and measured anthropometric and metabolic measures by the 6 months. Statistical analyses were performed using SPSS 16 software. Results: Patients with T2DM self-control regime were good (p < 0.05) among female patients. After 6 months diabetes education, patient's knowledge (P= 0.029) and self-control of blood glucose {p = 0.046) significantly improved in newly diagnosed patients with T2DM. Score of PAID reduced in educated group -2.86 (95% CI-l.6l-3.23, p = 0.004) and score of IPQ increased 8.95 (95% Cl 5.31-10.1, p = 0.001). After 6 months, the group of the educated patients had improved SBP (95%CI 144.5-100.9 mmHg (p -0.008), DBP (95%CI 99.7-71.1 mm Hg p = 0.035) and WC (95%CI 112.2-90.2 cm p = 0.014) were decreased. Level of HbAlC 95%CI 9.8-6.6%, p = 0.001 and LDL 95%CI 4.1-l.lmmol/L 0.001 were decreased. Conclusion: Patients with T2DM self-control of male and female patients shows that BP control (p = 0.041), SMBG (p = 0.003) and foot care regime were good (p = 0.001) among female patients. The diabetic patient's training environment, curriculum and the associated multidisciplinary professional team are insufficient. Outcome shows positive improvements statistically increased diabetes knowledge, self-management control, IPQ and PAID score decreased in the educated newly diagnosed patients T2DM.
