Three cases of idiopathic eruptive macular pigmentation

Idiopathic eruptive macular pigmentation is an under-reported condition of unknown etiology, characterized by asymptomatic, brown macules involving the trunk, neck, and proximal extremities in children and adolescents. We report three cases of this little known condition in two children and one adult. The first patient was a 19-month-old male presenting asymptomatic grayish patches on the face and trunk for 6 months. The second patient was a 3-year-old male presenting hyperpigmented patches on the trunk for 9 months. The third patient was a 27-year-old female presenting asymptomatic brown macules on the trunk for 1 year. In all of the patients, no pruritus, scaling, or erythema preceded the appearance of pigmented spots. The diameter of the macules varied from 5 mm to 40 mm, and Darier’s sign was not noted. The clinical diagnosis of these patients was varied as idiopathic eruptive macular pigmentation, lichen planus pigmentosus, urticaria pigmentosa, and erythema dyschromium perstans. Histopathologic findings of two patients showed epidermal basal layer hyperpigmentation, melanophages in the papillary dermis, and perivascular lymphocytic infiltrates. Given the clinical and histopathologic findings, a diagnosis of idiopathic eruptive macular pigmentation was made. No treatment was done because there were no symptoms such as pruritus.
