In recent years, the big trends in cancer therapy are targeted therapy and immunotherapy. While a growing number of new anti-cancer drugs have been used, various side effects have been reported. Secondary neoplasm caused by these drugs has also been reported, and diverse hypotheses on the mechanisms have been proposed. Ramucirumab is one of the new anti-cancer drugs inhibiting angio-genesis by binding VEGFR-2. In the present case, 40-year-old man was presented with erythematous nodule on the scalp and multiple erythematous tiny papules on the face and neck. The patient had history of advanced gastric cancer with multi-ple metastasis and received ramucirumab and paclitaxel treatment for four months. The exci-sional biopsy specimen of the scalp demonstrated crust on the surface, epidermal collarettes demarcating the lesion, dilated capillary vessel proliferation, and lymphohistiocytic infiltration in dermis. We diagnosed the scalp lesion as pyogenic granuloma, and the face and neck lesions as cherry angiomas. Herein, we report a case of vascular neoplasm after treatment with ramucirumab and the classic chemotherapeutic agent paclitaxel.
