Large superficial basal cell carcinoma successfully treated with ablative fractional laser-assisted photodynamic therapy

For the treatment of superficial basal cell carcinoma (BCC), not only Mohs surgery but photodynamic therapy (PDT) is an established treatment option. And the pretreatment of ablative fractional laser before PDT is proved to improve the treatment response by helping photosensitizers to penetrate the lesion. Here, we report two cases of superficial BCCs treated with an AFL-PDT. First, a 59-year-old male who was diagnosed with multiple superficial BCCs including several large lesions on the back, buttock and left arm. Second, a 49-year-old female diagnosed with large superficial BCC on the left forearm that occurred on the preivious burn scar. The large BCC lesions of two patients were decided to be treated with two sessions of AFL-PDT, and after the treatment, the lesions were almost cleared and there was no manifestation of recurrence after more than 3years of follow-up. Generally, BCC has a low malignancy but tends to have local destruction, so it is recommended to remove the lesion completely. And although Mohs surgery is usually performed for the treatment of BCC and has excellent outcome, AFL-PDT that is less invasive and has showed better cosmetic outcome can be used as an alternative treatment option for patients with superficial BCC for certain conditions like multiple large sites and burn scar beneath the carcinoma. In both cases, we revealed that AFL-PDT is useful and can be a better choice for patients with superficial BCCs.
