Treatment of pincer nail with topical hydrating nail solution

Pincer nail is a nail deformity characterized by increased transverse overcurvature of the nail plate. The overcurved nail compresses the subungual soft tissues of the distal nail bed, inducing pain and discomfort which limits the quality of life of affected patients. We report the case of a patient whose pincer nail deformity was successfully treated with a hydrating nail solution (HNS) (Evonail® solution, Evaux Laboratories, France). A 54-year-old Asian woman was referred to our department with a history of acquired pincer nail involving the second, third, fourth, and fifth left toenails, and second and third right toenails. HNS was applied once a day on all the affected nails using the application brush. After six months of daily application of HNS only, increased width and decreased height of the nail tip were observed resulting in significantly flattened nails. This result was well maintained, with no recurrence observed until ten months of follow up. We assume that by interfering with the signaling mechanism of substance P receptors, HNS solution results in anti-inflammatory effect on the damaged nails allowing the nail strength to overcome the mechanical force causing the nail deformity. Thus, HNS application could be a promising and non-invasive treatment option for pincer nail deformities, especially in cases with minimal pain which do not require prompt nail surgery.
