Objective: The aim of this study was to investigate the effect of Community Health Promotion Program on Fasting plasma glucose level and Blood Pressure Level of Village Health Volunteer with Prediabetes and Prehypertension. Methods: This study used the quasi-experimental one-group pre-post test designed to measure the effects of the intervention using Blood Pressure Level of Village Health Volunteer with Prediabetes and Prehypertension group. The Village Health Volunteer thirty in community-dwelling adults selected by means of purposive, qualification-based sampling were given a community health program based on Health Promotion Model of Pender et al. (2006) including 1. build healthy public policy and analysis of the situation related to health promotion individuals in the community. 2. create supportive environment and designing the health promotion program such as designating a space for exercise at the community.3. strengthen community action and implementing, reflection and revising the program. 4. development personal skills and the multidisciplinary team designed a health promotion program that consisted of the following activites, first, personal skills improvement by organizing a workshop to exchange the knowledge in community. 5. Reorient health services and conducted home visits. The Community Health Program developed by a multidisciplinary team and nurse case manager. Health status control in Fasting Plasma Glucose level and Blood Pressure Level tests was collected before and after the program in the twenty-fourth weeks. Descriptive statistics and paired t-tests were used to analyze the data. Results: After the twenty-fourth weeks, it was found that Health status control in Fasting Plasma Glucose and Blood Pressure Level test decreased significantly [ p-value < 0.01 ]. The study findings support the effect of the Community Health Promotion Program. Conclusion: The findings of this study proved that the Community Health Promotion Program is an essential component in the health promotion of Village Health Volunteer with Prediabetes Mellitus and Prehypertension group.
