Guideline updates in diabetes: hypertension

Hypertension is one of the most common comorbidities among adults with diabetes. Among adults with diabetes, the presence of hypertension has been associated with a two or three times higher risk for cardiovascular disease events and mortality. Given the number of guidelines regarding the management of hypertension in patients with diabetes mellitus, there remain differences between them as to what the BP threshold to initiate pharmacologic antihypertensive therapy should be, and, once started, the goal BP to achieve. The reason for differences is that we have only a few target-driven randomized clinical trials to test one BP target versus the other. Therefore, the blood pressure targets for controlling hypertension are under great debate in recent decade. Moreover, it is not surprising that there is no trials for Asian hypertensive patients including diabetes. Recently, Korean Diabetic Association issues new guideline for diabetic patients. This guideline provides a treatment and management of diabetes for healthcare practitioners or professionals on the basis of scientific evidence and clear benefits. This guideline also provides hypertension management in Korean diabetic patients including BP target. In this lecture, I will introduce this new Korean guideline especially about the hypertension management for diabetic patients. Furthermore, I will compare the guidelines from another countries and discuss the appropriate BP target for Korean diabetic patients.
