목적: Demonstrate a new instrument, technique or procedure
방법: The aim of this video is to show uterine artery preserving single-port total laparoscopic radical trachelectomy (RT) and demonstrate dissecting the uterine artery branches and preserving the ascending uterine branches. We used home-made single port system using Alexis wound retractor and a 7½ surgical glove. A 5 mm sized biopsy forceps with a slightly bent steel shaft on the left hand and 5 mm sized monopolar L-hook on the right hand were mainly used for uterine vessel dissection. Uterine manipulator (RUMI) was used to handle the uterus. After dissecting the uterine artery we further identified the uterine artery branches such as uterine, cervical, and vaginal branches. Then we ligated only cervical and vaginal branches of uterine artery and preserved ascending uterine branches. We also preserved bilateral round ligament to preserve the collateral vessels from the round ligament to the uterus. After completing dissecting uterine vessels and ureters the cervix was cut on the isthmic level and frozen biopsy on the resection margin was sent to pathology. Then colpotomy was performed with enough vaginal resection margin. Uterine cerclage procedure with 1-0 proline was performed on the isthmic portion while maintaining the uterine arteries. The uterus and vaginal cuff was approximated with interrupted suture (Vicryl 1-0) or continuous suture (V-Loc 1-0).
결과: We successfully completed 4 cases of single-port total laparoscopic RT from November 2011 to June 2016. There was no disease recurrence.
결론: Uterine artery preserving single-port total laparoscopic RT was feasible using convention laparoscopic instruments without any assistant port or any vaginal procedures.
