Background: Primary serous peritoneal carcinoma is a malignancy that originates from the peritoneum with typical features of diffuse peritoneal involvement and ascites. Curability is low unless the patient has a small tumor burden. Almost all cases of primary serous peritoneal carcinoma involve disseminated disease at diagnosis. Only a few cases of solitary Primary serous peritoneal carcinoma have been reported. We report a case of isolated peritoneal carcinoma without peritoneal carcinomatosis after total hysterectomy and bilateral salpingo-oophorectomy.
Case: A women underwent hysterectomy with bilateral salpingo-oophorectomy at age 51 for carcinoma in situ of uterine cervix. The ovaries and tubes showed no evidence of malignancy. Nine years later, she presented with a 5cm pelvic mass and underwent exploratory laparotomy with excision of the mass. Final pathology showed it to be a high-grade serous carcinoma consistent. The patient was treated with chemotherapy. There was no evidence of recurrence after 6 months of follow-up.
Conclusion: Isolated peritoneal carcinoma without peritoneal carcinomatosis and ascites is rare, Awareness of such unusual presentations of Primary serous peritoneal carcinoma should assist in the diagnosis of this disease, thereby improving the management of patients with this condition.
