Background: It is not uncommon to observe white hair regrowth (poliosis) in previous patches of alopecia areata (AA). However, studies of poliosis in AA are limited to case reports.
Objectives: This study aimed to identify the relevance between poliosis and AA.
Methods: Medical records of 258 AA patients who visited Wonju Severance Christian Hospital from March 2012 to June 2017 were retrospectively assessed. Demographic variables, comorbidities, and disease-specific variables were compared between “AA with poliosis” and AA without poliosis” group.
Results: Poliosis was observed in 13 patients (5.04%). The mean age, proportion of the patients treated with diphenylcyclopropenone and those with less extensive lesion were significantly higher in AA with poliosis group than the other group. Especially, in multivariate logistic regression analysis, significantly higher patients in poliosis group had a history of hypertension.
Conclusion: Although there is no pathomechanism that can directly explain the relationship between hypertension and poliosis in AA, mediators known to be involved in both vascular inflammation and follicular melanogenesis might be one of the suggestive evidence. Further studies with larger samples on the association of poliosis with other clinical factors are needed.
