Alopecia areata and itch: a retrospective analysis of 312 cases

Background: Patients with alopecia areata (AA) complain of various subjective symptoms with hair loss. Although itch is not rare in patients with AA, its importance has not been discussed well. Objectives: The aim of this study is to investigate the characteristics of AA with itch by comparing the clinical factors between AA with and without itch. Methods: Medical records of patients with AA who visited our clinic from October 2012 and May 2018 were retrospectively reviewed. The patients were subdivided into two groups, “AA with itch” and “AA without itch” based on the medical records. And clinical factors were compared between two groups. Results: Notably, 45 (14.42%) of 312 patients reported itch. Only body mass index showed a significant difference between the groups. But, there was a difference in the variables including smoking history, disease duration and serum white blood cell count that showed a significant correlation with itch according to the extent of AA. Conclusion: Given that mast cell secretion mediated by neuropeptide such as calcitonin gene-related peptide and substance P are involved in not only occurrence of itch but also the development of AA, itch may be a meaningful symptom suggesting the role of neuropeptide-mediated inflammatory reaction in AA.
