Nasotracheal Intubation in a Patient with Restricted Mouth Opening using a Video Laryngoscope and Cuff Inflation Technique: A Case Report

We report a case in which nasotracheal intubation (NTI) was performed by combined use of a video laryngoscope and a cuff inflation technique in a 50- year-old woman with limited mouth opening of 1.5 cm and restricted neck movement. Orotracheal intubation with a video laryngoscope was attempted, but there was no room in which to manipulate the endotracheal tube (ETT). We then tried NTI with video laryngoscope. After the ETT was inserted into the nostril, the video laryngoscope was introduced and the glottic inlet became partially visible with laryngeal compression. Instead of using Magill forceps, the cuff of ETT was inflated with air to lift it off the posterior pharyngeal wall, and the ETT was engaged into the glottis inlet and advanced into the trachea with the cuff fully deflated. Epistaxis or post-pharyngeal bleeding was not observed.
