Imiquimods are topical immunomodulators that are toll-like receptors-7 agonists. They induce the production of cytokines including interferon-alpha, a dose-dependent, antifibrotic cytokine. We experienced some fibrotic conditions successfully treated with topical imiquimod. In first case, a 54-years-old Korean female who had hyperthyroidism presented with multiple erythematous patches on pretibial area for 6months. We treated the lesion by 5% imiquimod cream and 1% tacrolimus oint and it have been much softened after 6 months of the treatment. In second case, a 57-years-old Korean female was diagnosed angiolymphoid hyperplasia with eosinophilia. Her disease showed a repetition of improvement and deterioration. We applied topical 5% Imiquimod cream and 0.1% tacrolimus oint. Skin lesions almost regressed after 6 months of the treatment. In third case, a 41-years-old Korean male presented with multiple well demarcated erythematous patches on face and both arms for 5 years. He was already diagnosed as generalized discoid lupus erythematosus. We treated lesions by topical 5% imiquimod cream with acitretin oral medication. Despite an initial development of erythema as a sign of drug-induced inflammation, entire lesions regressed in a remarkable manner after 9 months of the treatment. Herein we report our treatment experience and review the related literature.
