Riehl's melanosis is a nonpruritic pigmented dermatitis characterized by a brownish to grayish pigmentation that develops rapidly over the face, being most intense on the forehead and the temples. Among the various etiologic factors suggested, photoallergic reaction and pigmented contact dermatitis resulting from exposure to drugs, coal tar dyes, optical whitener, or other ingredients found in cosmetics are believed to be the major contributing factors in this disease. Histopathologic features of Riehl’s melanosis mainly consist of pigmentary incontinence along with infiltration of numerous dermal melanophages and lymphohistiocytes. While patients generally receive devastating psychological stress for the sudden darkening of their face, no single effective treatment has been identified yet. A few reports have addressed the efficacy of Intense pulse light (IPL) and low-pulse energy 1,064-nm Q-switched (QS) Nd:YAG laser about Riehl’s melanosis. Herein, we report the case of two Korean patients treated with a low-pulse energy 1,064-nm QS Nd:YAG laser for Riehl’s melanosis on the face and neck area.
