A case of epidermal cyst coexistent with trichoblastoma

A 57-years old female showed an erythematous elevated mass with central opening on her right occipital area. She underwent surgical excision under clinical impression of epidermal cyst. In the middle of operation, a different form of mass was detected at deep dermis and on excised specimen, well-demarcated yellowish mass with overlying cyst close to the epidermis was observed. Histopathologic examination showed a cyst lined by stratified squamous epithelium containing keratin material. Below the cyst, basaloid tumor was observed, which had peripheral palisading. No connection between cyst and tumor was observed. The patient was diagnosed with epidermal cyst coexistent with trichoblastoma and there was no recurrence for 2 years. Hair follicle is divided into 4 parts, and each part can develop into different types of tumor. The cells of each part can be mixed together and form a hybrid tumor. Epidermal cyst and trichoblastoma is a benign neoplasm derived from infundibulum and follicular germ cells respectively. Hybrid tumor which consists of epidermal cyst and trichoblastoma emanating from the cyst wall has been reported. Unlike previously reported tumors, both of epidermal cyst and trichoblastoma were found in upper and deep dermis separately without any connection in our case. Here, we experience a case of epidermal cyst coexistent with trichoblastoma and report with literature review.
