Granuloma annulare (GA) is a benign granulomatous disease with a different prognosis depending on clinical subtypes. While localized GA resolve spontaneously in a few months, generalized GA (GGA) is notoriously difficult to treat. Systemic hydroxychloroquine, neotigasone, phototherapy, and other various treatments have been proposed, but there are few patients who resistant to various treatment modalities. Herein we report 3 cases of GGA. These patients had been treated with systemic hydroxychloroquine, neotigasone, cyclosporine, and phototherapy, but showed recalcitrant course. Therefore, we tried methotrexate (initial dose 15mg/week) as alternative treatment. They showed nearly complete remission in 2, 4, or 4 weeks, respectively. Among them, 2 patients did not recur more than 6 months after drug withdrawal. Therefore, methotrexate could be an option for the treatment of recalcitrant GGA.
