Benign symmetric lipomatosis (BSL), also known as Madelung’s disease, fatty neck syndrome or Launois-Bensaude syndrome, is a rare condition characterized by extensive symmetric fat deposition in the cervicothoracic region. While the cause of the disease remains unknown, 60 to 90% of individuals with BSL have a history of chronic alcohol abuse. We report a case of BSL occurring in a 65-year-old man who has a 10-year history of the increase in size and numbers of asymptomatic, soft, subcutaneous masses on his posterior neck and back. He also has a medical history of acute pancreatitis and liver cirrhosis due to alcohol abuse. The biopsy specimen showed a mature adipose tissue without atypia, indicating lipoma. The patient was diagnosed with BSL based on clinical features and pathologic findings. The treatment would have been mainly surgical, but he wanted no further treatment. Herein, we report a rare case of BSL associated with increased alcohol consumption.
