연구목적 : 이 연구는 중증질환자의 민간의료보험 가입 현황을 파악하고, 민간의료보험 가입 여부에 따른 의료이용을 확인하여 이를 토대로 나타날 수 있는 문제점에 대해 대처방안을 논하고자 시도하였다. 연구방법 : 이 연구는 제7기 『2016년 국민건강영양조사』원시자료를 이용하였고, 19세 이상 성인 중 설문지에 결측치가 없는 536명을 분석 대상으로 하였다. 민간의료보험 가입 현황과 차이를 파악하기 위해 교차분석을 실시하였고, 민간의료보험가입에 미치는 요인을 확인하기 위하여 로지스틱 회귀분석을 실시하였다. 그리고, 민간의료보험 가입 여부에 따른 의료이용여부에 미치는 영향과 의료이용 횟수를 분석하기 위해 회귀분석과 독립표본 T-검정을 실시하였다. 연구결과 : 연구결과 연구대상자의 60.8%가 민간의료보험에 가입을 하였으며, 연령, 가구소득, 의료보장형태에서 차이가 있었다. 하지만, 민간의료보험 가입 여부가 의료이용 여부 및 이용 횟수에 미치는 영향은 없었다. 결론 : 연령이 많고, 저소득이며, 의료급여대상자에 해당하는 자 등 사회적 취약계층이 의료비 부담을 적게 하도록 국민건강보험의 소득재분배 성격을 강화하여야 한다. 또한, 중증질환자는 민간의료보험 가입 여부와 관계없이 의료이용을 결정하기 때문에 민간의료보험을 가입하지 못한 집단을 선별하여 건강관리를 위한 정부의 정책을 마련해야한다.

Objectives: The purpose of this study is to identify the status of private medical insurance participation for severely ill patients and to check their medical use according to whether or not to join private medical insurance. Methods: This study used the original data of the 7th "2016 National Health and Nutrition Survey" and analyzed 536 persons who were not missing in questionnaires among 19 years and over. Logistic regression analysis was conducted to identify factors affecting private medical insurance subscription. Independent sample T-test and regression analysis were conducted to analyze the effect of private medical insurance participation on the number of medical use. Results: 60.8% of the subjects were in private medical insurance. In the case of elderly, low-income, and medical aid, they were inclined to join private medical insurance. However, the effect of private health insurance coverage on medical use and frequency of use was not significant. Conclusion: The results of this study show that the severely ill patients decide to use medical care regardless of whether they have private medical insurance or not. Therefore, it is necessary to select the groups that do not have private medical insurance and to prepare government policies for health care.
