Sudden appearance of solitary brown macule on a 3-year-old boy`s finger

Candida parapsilosis is an yeast that can cause sepsis and infection in immune-compromised patients. It is usually isolated from animals, insects or soil but it is also commonly isolated from human hands or subungual spaces. It can be associated with pathologic nail and skin lesions. A 3-year-old male patient presented with an intermittently painful brownish patch on left 3rd finger and it was found 3 months prior to the visit. He had no underlying disease or trauma history on lesion but had contacted with hamsters 3 months ago. Under the first impression of porokeratosis, skin punch biopsy was done on the lesion. In histopathological review, many hyphae were found on stratum corneum layer, which were consistent with candidiasis. PAS stain and GMS stain of tissue also showed positive results. Then fungus culture of skin lesion was done with SDA media and white to cream colored colonies which were identified as candida parapsilosis were isolated. Final diagnosis was cutaneous candidiasis and lesion was treated with topical sertaconazole. The lesion was cleared in 4 weeks of treatment and no recurrence has been reported till now. There was no domestic report of cutaneous Candida parapsilosis infection and a few reports of cutaneous Candida parapsilosis infection with previous trauma history in English published journal. Herein we report a rare case of cutaneous Candida parapsilosis infection without underlying disease or trauma history.
