Hidroacanthoma simplex is a benign tumor assumed to arise from the uppermost segment of the intraepidermal eccrine duct and the acrosyringium. Histologically, there are well-defined islands of glycogen-laden small and uniform cells in the corfines of an acanthotic epidermis. Malignant transformation of hidroacanthoma simplex has been reported and named malignant hidroacnathoma simplex or in situ porocarcinoma. Pigmented hidroacanthoma simplex and pigmented malignant acanthoma simplex are rare variant which contain symbiotic dendritic melanocytes and may be confused with melanoacanthoma or malignant melanoma. We are aware of only four reported cases of pigmented malignant hidroacanthoma simplex in the literatures. Herein, we report a rare case of pigmented malignant hidroacanthoma simplex occurred in a 78-year old female.
