Two cases of acute Graft-versus-Host disease after liver transplantation

The acute graft-versus-host disease (GVHD) after liver transplantation (LT) is a rare, fatal complication with a 1% prevalence. This disease is the result of a severe alloreactive immune response by the donor organ’s T lymphocytes against the recipient. The transplant recipient’s antigen-presenting cells trigger the activation of the donor organ’s T lymphocytes, resulting in an alloreactive T-cell response against recipient tissues under the influence of cytotoxic T-cells and inflammatory cytokines. There are multiple treatment options ranging from increasing doses of immunosuppressive drugs, the dose reduction or stopping, or the addition of new immunosuppressive drugs. However, the results are very poor, with high mortality rate. We report 64-year old man and 53-year old woman who underwent LT. The postopertaive period was uneventful with nomalization of liver function. But, after 1month, both patients had erythematous scaly patches on the trunk, palms and fever. Biopsy of skin lesions revealing interface lymphocytic infiltrate and vacuolar degeneration in the basal layer with apoptotic dyskeratotic cells also corresponded to GVHD. Herein, we report two cases of acute Graft-versus-Host disease after liver transplantation.
