Tumid lupus erythematosus(TLE) is a rare variant of chronic cutaneous lupus erythematosus, characteristically, showing erythematous, urticaria-like plaques in a photodistribution. A 54-year-old woman presented with a 1-year history of swelling and a decrease in range of motion in her posterior neck and upper back. There was no improvement in spite of physical therapies and various herbal remedies. Physical examination showed a diffuse, erythematous, indurated plaque on the posterior neck and upper back. And cutaneous wrinkling was not noted with pressure. Biopsy revealed a mild interface change of the dermoepidermal junction with a perivascular and periadenexal lymphocytic infiltrate in the superficial and deep dermis. An increased dermal mucin was confirmed with Alcian blue pH 2.5 stain. Laboratory studies including antinuclear antibody titer were within normal limits. A diagnosis of TLE mimicking scleredema was made. There is only one previously reported case of TLE presenting with these physical findings in the English literature. Herein, we report the case of TLE with a unique clinical presentation like scleredema.
