Idiopathic palmoplantar eccrine hidradenitis (IPPH) usually appears in healthy children with tender, erythematous nodules on the soles or palms. Therapy generally consists of bed rest for a few days and nonsteroidal anti-inflammatory drugs. However, a proportion of patients experiences recurrent episodes of IPPH, causing significant pain and limiting their daily activities. We report a case of a 26-year-old man with painful erythematous swollen patch on both hand and feet. As his symptom recurred once monthly, he visited orthopedics but could not find any bony abnormality. We performed skin biopsy on erythematous indurated patch on his right sole. Histology finding indicated neutrophilic eccrine hidradenitis. With 2 weeks treatment of oral colchicine, his symptom resolved and no relapse were found on follow- up. There are only a few case report of IPPH in adults which is not associated with malignant hematologic disease, solid cancer and chemotherapy. As conventional treatment for IPPH is known as bed-rest and topical steroids, colchicine can be a treatment option in recurrent IPPH of adults.
