Erythema multiforme(EM) is a rare mucocutaneous eruption considered to be a hypersensitivity reaction mostly caused by infections and drugs. Infectious agents associated with EM are herpes simplex viruses 1 and 2, Mycoplasma pneumoniae, parvovirus B19, hepatitis B and C virus. Varicella zoster virus (VZV)-related EM is rarely reported. Until now total 10 cases of VZV-related EM are reported including 1 case in Korean dermatologic literature. A 64-year-old woman presented with tender erythematous zosteriform grouped vesicles on the left flank for 5 days and asymptomatic erythematous macules developed on the whole body for 2 days. Tzanck smear with zosteriform vesicle was positive for virus infection. To know the character of the asymptomatic macules, skin biopsy was performed. Histopathological examination showed the vacuolar degeneration of basal cell layer and necrotic keratinocytes in the epidermis. She was diagnosed with herpes zoster, and also VZV-associated erythema multiforme. Antiviral agent and low dose-systemic corticosteroid relieved her skin lesions.
