Churs-Strauss syndrome (CSS) is a rare syndrome characterized by asthma, blood and tissue eosinophilia, and in its full-blown form, eosinophilic systemic vasculitis, along with necrotizing granulomas centered around necrotic eosinophils. A 42-year-old man with CSS who had been treated with steroid for several years presented with 1-month history of multiple asymptomatic erythematous papules on his chest and both hands. Physical examination showed acneiform eruptions on his upper chest and whitish papules on his thumb. Although their clinical finding appears to be acne vulgaris or steroid induced folliculitis, histologic findings for both sites show palisaded granulomatous dermatitis with collagen degeneration and neutrophilic infiltration, consistent with Churg-strauss syndrome. Although not included in the diagnostic criteria, cutaneous manifestations are frequently observed in CSS. As the skin is the most easily accesible site for histologic examinations, greater awareness of the cutaneous features would help in earlier diagnosis.
