2010년 3월 23일 버락 오바마 대통령이「환자보호 및 적정의료법(Patient Protection and Affordable Care Act, PPACA)」에 최종 서명함으로써 미국에서는 역사적인 의료개혁이 이루어졌다. 미국의 금번 개혁은 의료보장의 적용 원칙 및 보험자 규제방식을 근본적으로 변화시키고 보건의료체계에 대한 국가의 역할을 강조함으로서 미국 의료보장체계의 패러다임적 전환을 가져왔다고 평가받고 있다. 그러나 미국에서 제기된 의료개혁의 필요성이나 배경은 현재 우리나라가 겪고 있는 보건의료분야의 문제점과 다르지 않다. 이에 본 연구는 오바마 행정부 의료개혁의 핵심인 「환자보호 및 적정의료법」의 추진과정과 전략, 동법의 주요 내용을 중심으로 그 현황을 검토하여 우리나라와 관련된 시사점을 도출하고자 하였다. 「환자보호 및 적정의료법」의 주요내용은 건강보험 적용대상자 확대를 통한 의료보장 사각지대의 해소, 공적제도의 보장성 강화, 민간보험사에 대한 규제강화, 일차의료 개선과 의료의 효율성 향상, 질병예방 및 공중보건 강화 등이다. 이는 현재 우리나라가 안고 있는 보건의료분야의 문제점이기도 한데 미국의 개혁내용을 바탕으로 우리나라에 대한 시사점을 살펴보면 먼저 전 국민 건강보험제도 외에 공적부조제도인 의료급여제도의 개선을 통해 취약계층에 대한 보장성을 확대할 필요가 있고, 각 제도들의 보장성을 확대하기 위한 노력도 지속해야 한다.민간보험에 대한 규제 강화에 있어서도 민간보험사에 대한 규제가 전무한 우리나라의 경우 미국의 민간보험사에 대한 관리감독 강화의 방향성 및 세부 제도를 고려하여, 단일보험자인 공보험에서의 효율성 확대를 위한 방안을 고려할 수 있다. 또한일차의료를 강화하고, 의료의 효율성 강화 및 의료의 질을 향상시키기 위한 미국의 다양한 형태의 시범사업과 적극적인 정부지원에 주목하여 향후의 개선방안을 모색하는 것도 중요할 것이다.
The US health care reform bill was legislated on March 25th, 2010, around 100 years since Theodore Roosevelt took the universal health care as a presidential promise in 1912. Transparency and program integrity, 6. Improving access to innovative medical therapies, 7. Community living assistance services and supports, 8. Strengthening quality, affordable health care for all Americans. This article will review US``s health care reforms, and will review the main content of the Patient Protection and Affordable Care Act(PPACA).
