Vitiligo is an acquired pigmentary disorder, clinically characterized by depigmented macules caused by destruction of melanocytes in the affected skin. Phototherapy and application of topical corticosteroids are most commonly prescribed. Topical corticosteroids have been shown to be effective as monotherapy in vitiligo, especially in head and neck lesions, with response rates of 64% in children. However, the head and neck are areas with skin that is among the thinnest of the body, and alternate therapies that do not cause atrophy would be welcome. Topical tacrolimus appears to fill this need. We report a case of vitiligo showing remarkable repigmentation with only topical tacrolimus ointment.
