Sweet syndrome is a neutrophilic dermatosis which can be associated with malignancies, infections, systemic inflammatory disorders, and medications. It is characterized by the following features: tender erythematous plaques, nodules, or papules; prodromal symptoms such as fever, malaise, or arthralgia; and diffuse infiltrate of neutrophils in the papillary dermis. Only a few cases of Sweet syndrome associated with extreme infiltration of eosinophils have been reported. A 44-year old man presented with localized erythematous annular plaques and papules on neck and trunk for several days. The lesions tend to be pruritic at first, followed by pain. He had a history of asthma. He visited our clinic with the same lesion 4 month ago and diagnosed with Sweet syndrome through skin biopsy. He also had blood eosinophilia. Histopathologic analysis of lesion revealed and vacuolar alteration of the basal layer in the epidermis and upper dermis showed edematous change and patch cellular infiiltraion consisting of neutrophils, lymphocytes and a large number of eosinophils without any sign of vasculitis. Herein, we report an interesting case of Sweet syndrome with abundant infiltration of eosinophils clinically associated with asthma, which has rarely been reported in dermatologic literature.
