A case of lymph node metastasis of basal cell carcinoma of penis treated with imiquimod

Basal cell carcinoma (BCC) is a nonmelanocytic skin cancer that arises from basal cells. BCC usually develops on sun-exposed areas of the head and neck and occurs very rarely in genital region. The prognosis for most patients with BCC is excellent and metastasis of BCC occurs only rarely. Involvement of regional lymph nodes and lungs is most common. BCC on the genital region may metastasize more rapidly and frequently than the tumor at other sites. Herein, we present a case of a 75-year-old man who had mass on the left inguinal region. He had a history of basal cell carcinoma on the glans of penis, which was treated with imiquimod and cured 7 months ago. Sonogram revealed multiple enlarged lymph nodes in the left inguinal area. Histopathologically, there were tumor with peripheral palisading basaloid cells which were identical to the previous tumor on the penis. Lymphoid follicles were distorted by the tumor cell nests. Physical and histological findings of the penis revealed no abnormalities. He was diagnosed as lymph node metastasis of BCC of the penis. The patient was transferred to Hospital of Seoul national university for further treatment.
