Cutaneous metastases of internal primary tumors are uncommon. The overall incidence of cutaneous metastasis of visceral neoplasm is 5.3%, ranging from 0.7% to 9%. Typically, cutaneous metastasis of gastric primary isextremely rare; its incidence is 1.1% in the Caucasian population. Multiple subcutaneous nodules on the trunk is the most common pattern of such metastases, and the most typical location is the umbilical area. Metastases to the scrotum have seldom been reported. It is originated mostly from the kidneys and the prostate. A 65-year-old male with gastric adenocarcinoma with bladder invasion presented with scrotal swelling and some indurated plaques. Result of the skin biopsy demonstrated that the cutaneous metastasis originated from the stomach. The neoplastic cells formed somewhat irregular but relatively well-differentiated glandular structures in desmoplastic stroma that consisted of round cells with irregular conspicuous nuclei. Herein, we report a rare case of metastatic adenocarcinoma of scrotum of a 65-year-old male, originated from stomach, to highlight the need for considering cutaneous metastasis as a possible diagnosis.
