Trichilemmal cyst (TC) is common cyst that arise from hair follicle. Women are overwhelmingly more likely to be affected than men. The Patients often present with solitary tumor on the scalp, but multiple lesions can be encountered at times. When diagnosing TC, exclusion of proliferating trichilemmal tumor (PTT) is important because of its malignant potential. Unlike TC, PTT tends to be larger and form more lobulated mass. A 26-year old woman presented 4.5 X 4.5cm sized dome shaped mass on her occipital scalp for 7 years. Considering the size and duration of the tumor, PTT was suspected. However, histopathological finding showed typical features of TC. We performed total excision of the tumor and there was no recurrence so far.
