Nodular cystic fat necrosis (NCFN) is a distinct condition characterized clinically by mobile subcutaneous nodules usually on the trauma-prone areas, most frequently on the lower extremities and histopathologically by encapsulated fat necrosis with lipomembranous change. Although its etiopathogenesis is still unclear, it seems to be related to trauma and subsequent rapid vascular insufficiency and has also been reported in association with variable systemic conditions causing a vascular compromise. We report an interesting case of NCFN that occurred on both shins of a young soldier, at the sites repeatedly compressed by shoelace knots of combat boots and suggest that chronic, repeated local ischemia as well as rapid vascular insufficiency by a single traumatic event may be a causative factor of NCFN. Our case may provide an additional clue to understanding the pathogenesis of NCFN more clearly.
