An atypical presentation of eosinophilic pustular folliculitis

A 28-year-old male without any medical history arrived with severe eruption on his whole body, including his face, which has been persistent for over 1 year. Biopsy was done with the impression of eosinophilic pustular folliculitis, and the results revealed subcorneal pustule containing many eosinophils and intrafollicular, perifollicular and dermal inflammation with many eosinophils, which was consistent with eosinophilicpustular folliculitis. The patient showed little response to topical steroid and tacrolimus, and systemic cyclosporine was added, in which slight improvements were observed. Classical eosinophilic pustular folliculitis, or Ofuji disease, presents with recurrent follicular papules and pustules predominantly on the seborrheic distribution. The case we report shows an atypical and severe presentation with a distribution on his whole body.
