Foreign body granulomas caused by hyaluronic acid filler injection

Hyaluronic acid (HA) fillers are popular agents for filling wrinkles caused by aging. The advantages of HA fillers are long lasting effect, less immunogenic component. With the popularization of HA fillers, filler-related complications such as nodular mass, inflammation, necrosis and foreign body granuloma (FBG) are increasing. A51-year-old woman presented with erythematous nodules and swelling on the right periorbital area and around both marionette folds. She reported having 2 sessions of HA filler injection on the both sides of periorbital areas, nasolabial folds and oral commissures 10 and 2 months prior to presentation. Histopathological examination from the right marinonette fold showed a bluish material surrounded by epithelioid histiocytes and multinucleated giant cells with infiltration of lymphocytes in subcutis. From these findings, patient was diagnosed with FBG to HA filler. After 2 weeks, right infraorbital area, both cheeks along the nasolabial folds and chin became erythematosus and swollen. She was treated with a few times of hyaluronidase injection and lesions improved considerably. Although formation of FBG on injection site has known to be common complication, in this patient, FBGs developed in non-injection sites as well as injection sites. It assumed that downward migration of HA filler caused FBG. Here, we report a case of FBG caused by HA filler occurred on not only injection sites, but also non-injection sites.
