Onychomatricoma is a very rare benign tumor of the nail unit. Very recently, we suggested that onychomatricoma might derive from onychodermis, a specialized mesenchyme containing onychofibroblasts below the nail matrix and nail bed. A 65-year-old man presented with a 3-year history of a fingernail dystrophy. Clinical and dermoscopic examination shows multiple longitudinal cavities with keratinous material. Nail clipping was done for histological diagnosis. Based on clinical and histopathological findings this case was diagnosed with onychomatricoma. Recently, it was reported that analysis of nail clipping is a fast, minimally invasive method to achieve the correct diagnosis of onychomatricoma, which shows a thickened nail plate with multiple cavities in H&Esections. In our case, multiple milium-like lesions at the distal nail plate margin were observed in the dermoscopy. This dermoscopic finding seems to be pathognomonic for onychomatricoma. To the best of our knowledge, it is the first case of onychomatricoma that was reported ever.
