The aim of this paper is to report a case of 24 year old man who had a plastic nasal filler injection with cosmetic purpose. The man who has no history underwent complication of a filler injection and visited the emergency room. He showed left hemiparesis, OD visual disturbance and right MCA territory on MRI. We gave TPA, but cerebralhemorrhage was found on brain CT for checking deep drowsy mentality. So anoperation (1.decompressive creniectomy hematoma aspiration) and conservative treatment made up alert mentality The filler is injected into the skin or a subcutaneous layer in order to correct or reform wrinkles, the recessed region, acne scars and facial contours. Many people can easily access and find this treatment for cosmetic purposes in recent years. But this case is a rare case of filler injection complication.
