Is vaginal cytology and CA-125 measurement useful to detect recurrent endometrial cancer in postoperative patients ?

목적: To evaluate the role of follow-up after surgical treatment in endometrial cancer, we investigate predictive factor (vaginal cytology, CA-125, physical exam) for detect recurrence. 방법: 345 Patients undergoing primary surgery with final pathology consistent with endometrial cancer between 2000/1/1-2014/5/1 were identified. We retrospectively studied these patients who had a follow-up appointment consisting of vaginal cytology, CA-125 and imaging which were evaluated by univariate and multivariate analysis. when we receive a diagnosis of recurrence were evaluate CA-125 measurement, vaginal cytology, physical exam, for recurrence group. non-recurrence group was performed those test every 3 month or 6 month. 결과: 43 patients relapsed in 345 endometrial cancer patients, the median time from surgical treatment to recurrence was 18.25 month (range 1~78 month). early stage (I,II) in recurrent group was 22 patients and advanced stage (III, IV) was 20. We found most to detect relapse by imaging study (routine PET, CT) 24 (55.8%) in 43 patients who had recurrence and CA-125, vaginal cytology, physical exam found each 11(25.6%),0,8(18.6%) patients. CA-125 is significantly higher (Pearson ×2 = 57.5, p value<0.001) than the other tests, vaginal cytology(Pearson x2 = 14.13, p value<0.001) , Physical exam(Pearson ×2 = 55.37, p value<0.001), in correlation to the recurrence endometrial cancer. 결론: We found PET-CT and abdominal CT most contributed to detect recurrent endometrial cancer. in fact, abnormal vaginal cytology and CA-125 elevation are not significant predictive factors suggesting recurrent endometrial cancer than imaging study. but follow-up using these methods with follow-up imaging, it may helpful to detect recurrence.
